Wisconsin's SSM Dean Medical 2 Embrace OpenNotes

OpenNotes is an international movement supporting the adoption of open notes.  This is a fundamental change in the practice of medicine in Wisconsin. 

This is a health care quality collaboration - where patients can see all the notes that are generated from contact with the SSM Dean Medical Delivery System.  This is an improved use of electronic medical records. 

In July of 2017 patients will be able to see their visit notes through Epic's MyChart portal.  Early in 2018 sharing of this information with the patient is expected to become automatic. 

This activity is about fostering efficiencies across a network of both medical and non-medical providers.  The medical system is serving an older population with newer costly treatments.  The delivery process needs to remove the separation to facilitate better outcomes. 

This means Home Care Path senior support partners will be working with elders to help them access the information from their medical visits.  The expectation is that family and supports will assist the elder to participate in the medical provider instructions that will improve wellness.

Home Care Path expects the Medical Clinic to eventually remind the patient to electronically review the visit notes when they exit the building.  Network efficiencies involves encouraging supportive home care organizations to identify tasks that improve wellness beyond the walls of the medical delivery facilities.
