Home Visits Shifting Community Perception

The State of Wisconsin has developed the IRIS program, the Family Care Program, and the Wisconsin Partnership Program, which serves nursing home level of care patients who choose to reside in the community.  This population receives home visits to maintain wellness.

Local hospitals have placed health care services in the same geographical location and call themselves health care centers.  These providers now build large outpatient clinics to serve the area citizens.  Following an outpatient procedure many individuals benefit from the help of another (home visits) for a period of time.

A recent survey conducted by the Health Care Intelligence Network www.hin.com of New Jersey found that while 79 percent of the hospital health care system conduct home visits only 0 to 10 percent of the total population receive a home visit.  With a Federal, State, and Health Care Industry shift toward serving more individuals in the community the 0 to 10 percent reported figure should begin to climb.

Forty three percent of the respondents report post discharge as the primary reason for a home visit.  This survey involved 155 organizations.   The data indicates an individual is selected for a home visit by a process that includes chart reviews, electronic health care records condition program, hospital census, and self reports.

Home Care Path is a community based organization committed to facilitating the integration of in home supportive care with public funded programming and medical provision.  This involves supportive care solutions to guide the varied insurance plans implementation of health benefits near the point of service.  This is an effective way to manage loss through reimbursement tied to : preventable readmissions, repetitive ER visit loop, outpatient observational stays, and funding skilled medical staff for non -medical tasks.

Insurance Exchange and Minimum Value Standard

Under the Affordable Care Act the employer sponsored health insurance plan sold to employees must meet a Minimum Value Standard.  An employer sponsored health plan meets the "minimum value standard" if the plans share of the total allowed benefit costs covered by the plan is no less than 60 percent of such costs (Section 36B (c)(2)(c)(ii) of the internal revenue code of 1986.  Health and Human Services rules must be utilized for determining minimum standard value.

Minimum value is calculated (HHS MV Calculator) by dividing the anticipated covered medical spending of essential health benefits (EHB) coverage (for the population covered by a typical self insured group plan) by the total anticipated allowed charges for EHB coverage for a typical self insured group health plan population.

This Minimum Value Standard (actuarial value) measures the relative generosity of benefits covered by the health insurance plan.  Under the ACA Affordable Care Act a health insurance plans actuarial value (60% is minimum value standard) indicates the average share of medical spending that is paid by the plan, as opposed to being paid out of pocket by the consumer.

So, we can see the actuarial values (60% is minimum value standard) are used to categorize insurance plans in to health benefit tiers.  See below:

Bronze = 60%  (this is the minimum value standard)

Silver = 70%

Gold = 80%

Platinum = 90%

Gold of course being more generous than bronze.  An employer group who purchases an insurance product through a work setting would need a plan that delivers a 60 percent contribution to the total share of medical spending.

The Commission on Long Term Care Report is Published

The Commission on Long Term Care which was formed June 10, 2013 under a law signed January 2, 2013 has published a final report http://www.ltccommission.senate.gov/Commission%20on%20Long-Term%20Care%20-%20Final%20Report%20-%209-18-13.pdf.  The final report is required by Section 643 of the American Taxpayer Relief Act of 2012 (P.L.112-240).

The Commission on Long Term Care was formed to examine long term care services and supports in areas of provision, staffing, and viable pay source.  This period of time is unique in that the largest generation in history is growing old. The data indicates over 12 million Americans of all ages need long term care supportive service.  This need will grow dramatically over the next 2 decades.

Financially the only way to make an insurance product that can effectively serve this population is to spread the risk over a huge base of plan holders.  The sustainable structure for this insurance product is currently not available.

The Commission describes the future of Long Term Care Services and Supports as being integrated with public programming and the medical providers.  The Commission indicates community based supportive care service providers ( www.homecarepath.com) will need to participate in outcomes focused care management.  Home Care Path staff discuss this approach as the shift from selling hours to developing a safe plan that meets the individuals needs.

Home Care Path as a Wisconsin long term supportive care provider develops a plan of care with the consumer, caregiver, family, and medical staff in mind.  This inclusion improves participation in person and family centered care.   The ongoing updates delivered to Home Care Path staff and family through newsletters, web site, blog, App all improve access to caregiver information.

Home Care Path staff receive ongoing training, which includes a manual (written updates) and in home training, with additional help as conditions change.  Home Care Path flexible care plans (inclusion of family) and no scheduling minimum enhance family and care giver engagement.  Including the medical providers in the care plan strengthens integration and allows families the opportunity to deduct the cost of the services from taxes in some situations.

The Wisconsin PATH ALONG model includes staff trained to provide supportive care services for an individual throughout the variation and physical locations of the medical delivery offices, clinics and hospitals.  As the medical system updates the community facilities to provide treatment to an aging population with chronic disease processes PATH ALONG staff can attend to the supportive care needs.

Home Care Path and the Wisconsin PATH ALONG model work with available Federal, State, and Military benefit specialists to establish a service that provides the person and family an opportunity to maximize utilization of the available public programs.  This can involve facilitating access to participating in organization of the data required to maintain service.  The Commission on Long Term Care Final Report is published and the work continues.

VA's Fully Developed Claims (FDC) Program

Using the (VA) Veterans Administration's Fully Developed Claims Program participants can get a faster decision (about 120 days) on their submission.  A passed FDC incentive bill facilitates retroactive compensation up to one year from the filing date.  This helps insure no loss of benefits while collecting and organizing the required documents that certify the claim as fully developed.

There are 2 reasons for filing a Fully Developed Claim with the VA.

1. You have an injury, disability, or condition that occurred or was aggravated by Military Service.

2. You have a condition caused or aggravated by an existing Military Service related injury.

The way to file a Fully Developed Claim with the VA.

Connect with a Certified Veterans Service Officer (CVSO) to help you initiate the claim
(eBenefits.va.gov ), gather the required medical records and documentation and submit your claim.

As the program is utilized the focus is to have the veteran provide the Veterans Administration  everything needed to deliver a fast claim decision.  This new process is also expected to improve the Veterans ability to get the claim awarded.  The goal of the program is to get Veterans the benefits they are entitled to in the fastest possible time.

What Am I Buying Health Insurance Marketplace

Understanding what the health insurance industry will deliver with the plans being marketed has to do with EHB.  Essential Health Benefits (EHB) describes 10 services that must be covered with health insurance.  The (ACA) Affordable Care Act ensures health insurance offered in group and individual markets both inside and outside the Health Insurance Exchange Marketplace, provide a comprehensive package of items and services, known as Essential Health Benefits.

This creates a standard , uniform structure for all health plans.  This improves the buyers ability to understand what they are receiving when they purchase the health insurance plan.  This gives the clinics, hospitals, and nursing homes a clear idea of the services that will be reimbursed post provision.

The 10 Essential Health Benefits List

1. Ambulatory patient services, this is outpatient care without being admitted to the hospital

2. Emergency services

3. Hospitalization

4. Maternity and  newborn care

5. Mental health and substance use disorder services, including behavioral health treatment, this includes counseling and psychotherapy

6. Prescription drugs

7. Rehabilitative and habilitative services and devices to help people with injuries, disabilities, or chronic conditions gain or recover mental and physical skills

8.  Laboratory services

9. Preventive and wellness services and chronic disease management

10. Pediatric services.

The health insurance plans now afford families a broader menu of needed services.  No more worry about what might develop over time and trying to intuit how to scrounge resources to help.  When the unexpected need presents itself, the health insurance plan can support the family through the treatment and ongoing recovery process.

Having a way for families to effectively meet an unexpected health need is believed to be a primary force for improving productivity and facilitating a stronger economy.  Home Care Path encourages readers to follow this issue as it unfolds in the next few months.

Renewal Offers For Existing Health Plans Are In The Mail

You have all experienced the terrifying scary information to facilitate a perception of bad (coming premium increase) health insurance news.  The Affordable Care Act requires health insurance plans to deliver essential health benefits that will likely cost more money.

To help you minimize this fear the media hype has created- existing health plan holders will receive a renewal choice offer in the mail.  The insurance company will encourage policy holders who would normally renew in January 2014 to September 2014 an opportunity to lock in at a lower rate by December 2013.  You are renewing December 2013 to avoid rate increases anticipated with the Affordable Care Act in 2014.

Say you carry a catastrophe plan with high deductible for about 132.24 per month.  The renewal choice offer may contract at 150.07 per month for one year for similar benefits.  This right out of the chute offer lacks an obvious point of comparison the Federal Health Exchange Marketplace has taught us we so desperately need.

October 1, 2013 will begin the busiest annual policy renewal season in the health insurance industry.  Any consumers that can be locked in (served and retained) by December 2013 through a renewal choice offer lessens the labor required to serve this influx of changing plans.

Most health care consultants would advise you to check the marketplace to determine eligibility for subsidized coverage and tax credits, prior to making a choice.  This would involve having access to the information needed to make the best choice for yourself and the family.

Home Care Path encourages readers to continue to follow this issue as it unfolds in the next few months.

Want Wisconsin MA Watch Your Assets

Let the reclamation begin.  The Wisconsin Budget Committee voted 10 to 6 to define a standardized approach to recovering assets from estates of former MA (Medicaid) recipients.  This is another way of saying the medical assistance benefit is becoming tighter.

Wisconsin is defining all property as marital property at the death of the community spouse.  This gives the State of Wisconsin the means to lay claim to all property the couple owned up to five years before the MA application was filed.

The State of Wisconsin wants to claim the property at the death of the second (community spouse) spouse to pay back the Medicaid received by the institutionalized spouse.  The inclusion involves all property owned by either spouse in a marriage at the time of the MA (Medicaid) application.

Estate planners suspect this may change the way individuals form contracts (marriage) to couple.   Retaining visible assets in a family home where a parent has a disabling medical illness just got a bit tougher in the State of Wisconsin.   Home Care Path encourages readers to watch this issue as it unfolds in the next few months.

PATH ALONG model affects hospital readmissions

Wisconsin's PATH ALONG model integrates supportive care service in to the evolving health care system.   Emerging new models are essential as an aging population needs both medical care and supportive service to maintain the best quality of life.

Wisconsin's PATH ALONG model performs a comprehensive application of discipline specific care transition instruction.  This is a collaborative position that improves the allocation of the vast array of care options near the point of service.  More comprehensive coordinated services in the inpatient delivery reduces the need for Medicare reimbursed services and Medicaid financed nursing home care.

The Wisconsin's PATH ALONG model places need and timeliness of service on the same discharge  patients plate as fragmented provider locations and varied payers.  This improves outcome.  Wisconsin's ability to adapt new models as a way to meet the needs of an aging population is an important political, economic, medical, spiritual, and social imperative.

A model that includes these supportive care needs that are more prevalent with advanced age will have a positive impact on health, well being, and over all medical costs.   Home Care Path (www.homecarepath.com) encourages readers to watch this issue as it unfolds in todays health care delivery.

Link Discontinuous Health System Changes

People are living longer.  The hospital today is treating more chronic exacerbations than acute conditions.  The chronic disease responds better to ongoing management than an occasional treatment visit.  So, health care delivery has shifted from acute to a chronic treatment process.

Chronic disease requires ongoing management which works best when the population being served has a stable pay source.  This involves a greater integration of publicly collected funds being legislatively budgeted to supplement the health insurance expansion.  More insured can mean more users.

With the increased public money comes a more comprehensive utilization over sight.  Expect to experience a tighter benefit at the point of delivery.  Simply , no more paying skilled medical staff to perform non-medical tasks.

Along side this is a top down approach to encouraging more care to occur in the community.  The State of Wisconsin has implemented the Wisconsin Partnership Program, The Family Care Program, and the IRIS Program to facilitate the delivery of long term care in to the community and out of the institutional setting.

Many rural Wisconsin hospital's have built (health care center) out patient clinics on the property.  This is a cost containment approach, which moves acuity down the care continuum.  Many medical procedures can be performed outside of the hospital in the out patient clinic setting, and cost the health insurance plan less.

To efficiently serve a larger number of (patients) users and demonstrate a fiscally responsible approach to reimbursable claim submission the hospital provider will benefit from the help of a community based supportive care organization.

Hospital staff recognizing the patients need does not meet a skilled medical requirement means an immediate contact with a supportive care organization (www.homecarepath.com).  To effectively fulfill the care assurance role, hospital staff will complete a timely referral for the patient to a supportive care provider.

This will be incorporated in to the inpatient staff training sessions as a way to demonstrate the effort to manage loss secondary to :
1. preventable readmissions
2. repetitive ER visit loop
3. outpatient observational stays
4. funding skilled medical staff for non-medical tasks.

So the health system changes will involve closer communication across inpatient provision of hospital treatments coupled with this greater dependence upon supportive care providers serving the local community.

Wisconsin 2014 Medicaid Gap Health Insurance Exchange

In January 2014 the Federal Affordable Care Act regulates the minimum Medicaid income eligibility level across all the United States.  Any individual under the age of 65 with an income total below one hundred and thirty three percent of the Federal Poverty Level will be eligible for the Medicaid benefit.

This means an individual earning less than 15, 282.00 annual income will qualify for Medicaid without the need for a waiver.  This is contingent upon your State of residence participating in the Federal Medicaid guidelines.  Leaders in the State of Wisconsin have not developed the infrastructure needed to comply with the approaching implementation of Federal Medicaid regulations.

What this currently means in Wisconsin is an individual with an annual income level of 100% of the listed Federal Poverty Level 11, 490.00 would be eligible for Medicaid.

An individual with an annual income between 101 % and 133% of the listed Federal Poverty Level 11, 491.00 to 15, 282.00 would be sent to see a Health Insurance Exchange Navigator to pay for a health plan.

So, the defined Medicaid gap in the State of Wisconsin appears to be those with an annual income of 101 percent (11, 491.00) to 133 percent (15, 282.00).  Under Federal Law this population is eligible for Medicaid.  But in the State of Wisconsin this population will be denied Medicaid and instructed to purchase an insurance plan on the Wisconsin Health Insurance Exchange.

For an individual this translates to 3792.00 annually, or about 316.00 per month of household income.  This appears to be money that Wisconsin house holds at this income level would need the most.

Interestingly the Federal mandate requiring individuals to have health insurance does not apply to the Wisconsin people who's annual income falls between the 101 percent to 133 percent.  If you qualify for Medicaid at the federal level you cannot be forced to buy insurance in a state that opts out of Medicaid expansion.

This identifies a population (Wisconsin Medicaid Gap) that will have to be served by community hospitals but may be without a valid pay source.  Home Care Path encourages readers to follow this Health Insurance Exchange implementation as it continues in Wisconsin.

Wisconsin Health Insurance Exchange and Plan Comparison

The Health Insurance Exchange Marketplace is about comparing plans to determine the best  purchasing deal for your family.  Individuals who get insurance through an employer can still purchase a plan on the Health Insurance Exchange.

Find out if you qualify for subsidies.  This could involve a premium tax credit which would lower your monthly premium.  Qualifying for cost sharing subsidies can reduce your deductibles and copays.

If you are enrolled in an employers plan now, is the coverage considered adequate and affordable under the law.   If the health insurance you now purchase through an employer is considered adequate and affordable you would not be eligible for the subsidies that reduce the cost of the exchange plan.

By comparing your existing plan with the plans on the exchange marketplace you will have a comprehensive perspective as to the value of your eligible health insurance benefits.  Moving in to a health insurance product that delivers a better over all fit for your families situation  is getting easier.

Home Care Path encourages Wisconsin families to follow the implementation of the Health Insurance Exchange over the next few months.

Molina Health Care Inc NYSE MOH 34.27 Take Off Wisconsin

Molina Health Care Inc NYSE MOH 34.27 is the one to watch.  Molina has a long history (founded in 1980) of insuring low income programs.  Last year Molina Health Care generated about 70.7 million dollars of premium revenue in Wisconsin.

Molina applies its systems to generate profit beyond what the standard State contract has agreed to pay the managed care organization.  Participating in Medicaid and Medicare advantage plan markets, and now on the Wisconsin Health Insurance Exchange.

Molina takes an expansive approach to managed care by developing and operating internal design to create revenue flow.  For example, Molina has begun developing and opening  primary care clinics in the States it serves.  Staffing care provision within the utilization management arm of the health plans explanation of benefits can reduce undesired variables in the over all systems flow.

From the outside looking in, one would suggest Molina Health Care Incorporated delivers a service with an expansive quality that translates to bringing more care under management.  Having more care under management stabilizes the margin (profit margin) between pay source and medical provider.

This system stability enforces the community perception that the delivery of the needed medical service will be available when it is needed.  Home Care Path wishes Molina Health Care Inc another great year of conducting business in the State of Wisconsin.

Biggest Financial Threat Wisconsin Hospitals Face

The impact of an aging population, specifically the chronic disease processes present the single biggest threat to hospital finance.  Hospitals nation wide are being encouraged by Medicare policy to reduce inpatient readmission rates through closer attention to care transitions.

People are living longer.  The chronic disease processes accompany advanced age.  Research indicates chronic disease responds better to ongoing management than occasional treatment.  Ongoing management involves the sharing of a patient's needs between providers.  Care transitions is the sharing of a patient's needs between providers.

Hospitals are being challenged to add value to their care delivery and documentation process by linking with community based organizations (www.homecarepath.com) that give secure participation in the varied transitions present with an inpatient discharge.  Hospitals will tighten up and strengthen communication with community based organizations to manage potential loss related to Medicare cost containment policy.

Attaching a valid pay source to Wisconsin individuals who's income ranges from above 100 percent to 200 percent of the Federal Poverty Level may include a larger investment in community (labor) coordination.  The County determination process may indicate through the functional eligibility screen the person is eligible, but financial (over assets)  income can send the individual to the health insurance exchange navigator for help.

Connecting more Wisconsin users of the health system to a valid pay source helps medical providers better manage the undesired symptoms that accompany the chronic disease processes. Home Care Path encourages readers to watch this issue as it unfolds over the next few months.

Debt Collection Offices Interested in Wisconsin Health Insurance Exchange

Debt collection offices have been interested in the Wisconsin Health Insurance Exchange Marketplace because of the phrase "reasonable efforts".   In compliance with the Internal Revenue Service 501 (r) non-profit hospital's must make reasonable efforts to see if a patient is eligible for financial assistance before processing a debt through a collection agency.

The non-profit hospital can still sue a patient for an amount owed, but only after thoroughly demonstrating the patient is not eligible for financial assistance.  The hospital must provide for the patient 120 days, along with the forms and help if needed to procure available funds for the medical bill.

The claims department will be asked to demonstrate how all reasonable efforts have been taken to ensure the individual did not qualify for financial assistance prior to the hospital employing a collection agency.  So, simply posting a notice in the waiting room would not be sufficient under this regulation.

Now, more than ever it is in the hospital's best interest to work with community organizations (www.homecarepath.com) that can assist families with transitioning from private money to publicly funded programs.  Assisting community members to access service through organizations authorized to process public funded program determinations.

Implementing the new criteria under Federal law can take time and effort on the infrastructure within the individual States.

Will Wisconsin have the community processes in place to insure eligible individuals are receiving access to this Medicaid benefit.  Shaping County services to afford Wisconsin an opportunity to comply with the Federal law is in practice.

Realizing how the Medicaid benefit is transitioned from the County office in to the Wisconsin Partnership Program, Family Care Program and IRIS Program for care management demonstrates the economic exchange.  Medicaid dollars pay employees in the County and the Wisconsin health programs, who then spend the money in the community on their own family needs.

This bolsters the individual States economic activity.  Understanding how the Medicaid process contributes to the communities economic activity can improve participation in ongoing planning.  Home Care Path encourages readers to follow this health care issue as it continues to unfold.
