New View Of Care Coordination To Reduce Readmissions

Readmission to a hospital setting within 30 days of discharge adversely effects public (Medicare) funded reimbursement.  Hospital providers know readmissions are no longer an accepted part of the medical practice.

Senior Likes Micro Fleece Texting Gloves In Wisconsin

Wisconsin Ice Fish Without A License Weekend Jan 18 and 19

Seniors have reminded us the second year of the free ice fishing weekend is approaching.  Everyone can fish in Wisconsin waters on January 18th and 19th- 2014, without a fishing license.  This is a great way to experience your favorite lakes and catch some fish.

Seniors remind us to always ice fish with more than one person for increased safety.  Warm clothing, snacks, and warm fluids all improve the experience.  Remember snow can conceal thin ice, so fish in areas often frequented by local ice fishing families.

Can Anklebot Help After A Stroke

Anklebot can be described as a bio scanner and walking coach device.  Robotics is mounted to a knee brace and connected to a custom designed shoe.  Electrode sensors pick up when the person begins to move their ankle on there own, and adjusts the robotics to deliver less assistance.

The anklebot is a product of The Newman Laboratory for Biomechanics and Human Rehab at (MIT) The Boston Mass. Institute of Technology.  The (VA) Department of Veterans Affairs Journal Of Rehabilitation Research and Development has published  an initial study involving post stroke ankle stiffness and gait function with inclusion of the anklebot.

VA Researchers Larry Forrester and staff will conduct a 2 year study to document how the anklebot can help stroke survivors improve their gait.  As a person moves the ankle, sensors trigger robotics which move the foot along the programmed trajectory.  Electrode sensors record angular displacement and torque as collected data on a computer program.  Researchers can use the data to determine ankle stiffness and (ROM) range of motion.

Researchers hope to publish scientific evidence for the ability of anklebot to improve gait velocity and forward propulsion . With the completion of this study the expectation will be the inclusion of anklebot in to the rehab regimen for the post stroke population experiencing difficulties with ambulation.  Most importantly the post stroke patient can have access to a simultaneous therapy that could help them walk better.   Home Care Path encourages readers to follow this new therapy practice as it becomes more available.

CMS Calculates 18 Percent Hospital Readmission Rate 2013

Home Care Path and the Wisconsin PATH ALONG model were pleased with the recent hospital readmission rate data reported by The Centers for Medicare and Medicaid (CMS).  CMS Data Analytics reviewed 30 day all condition Medicare readmission rates.  Focusing on January 2012 to August 2013 preliminary figures indicate the Medicare readmission rate averaged less than 18 percent.

This is significant when compared to readmission data from 2007 to 2011 which reported a 19 percent Medicare hospital readmission rate.  This one percent drop is estimated to mean 130, 000.00 fewer hospital readmissions the Medicare fund needed to reimburse.

The national effort to manage hospital readmission rates is called (CMS) The Centers for Medicare and Medicaid value based purchasing.  This calls on Medicare certified providers to deliver the assessed need within the appropriate level of care setting.

This means the Medicare and Medicaid fund will only reimburse a certified provider group at the amount pre -defined according to where the treatment can safely occur.  So, if your scheduled procedure has been proven to be safely done in an outpatient setting, the Health Care Center will not be reimbursed at the inpatient hospital level.

This in part is why you are seeing the construction occurring near the community hospitals.  This is adding the outpatient specialty delivery areas.  With these medical services the patient comes early has the procedure and goes home the same day.   The Medicare cost is much less for a procedure that does not include a stay in the hospital.

Wisconsin Embraces Alternative Therapy With Kwekkeboom Study

Kristine Kwekkeboom a Wisconsin Registered Nurse has organized a progressive approach to symptom management.  Funded by a National Institutes of Health Research Project Grant for 2,022,591.00 to be conducted in cooperation with the University of Wisconsin Madison. 

For approximately 4 years the study will document significant variables to better understand (evidence based) how and for who the interventions will work best .  The interventions are non invasive and support an internal locus of control.  The therapy is employed to help manage pain, fatigue and sleep disturbance.

The technology is an MP3 player, which is a hand size, portable device that stores digital audio files which are usually songs and music. The MP3 player will give the study participant immediate access to audio files recorded as relaxation, distraction, and guided imagery content.  The MP3 device delivers access to their chosen selections any time and any place the symptoms become intolerable.

Survivors Benefits Social Security Administration

A community members death is communicated to the Social Security Administration through the (I-EDR) Internet Electronic Death Registration by the chosen Funeral Director.  The (I-EDR) Internet Electronic Death Registration replaces the need for the Funeral Director to file form SSA-721 with the Social Security Administration.

Form SSA-721 was a statement of death filled in by the Funeral Director.  Electronic posting of this information enables the Social Security Administration to deliver benefits much faster. Two important benefits for a surviving spouse include:

Lump sum death payment: The Social Security Administration distributes a special lump sum death payment of 255.00

The Social Security Administration can re-calculate the couples benefit to determine if the surviving spouse will benefit from drawing the decedents funds.

Today the police dispatcher sends EMS rescue who perform a bedside diagnostic test which confirms the individual has no pulse or respiration.  The EMS rescue contact the coroner who declare the time of death and contact the funeral director.  The funeral director notifies the Social Security Administration as a statement of death through the I-EDR  which enables the surviving spouse faster access to needed benefits.

Chevalier of the Legion of Honor

The Legion of Honor is the highest decoration in France and Chevalier is the highest degree of the five that can be awarded.  The Legion of Honor is a French Order that was publicly established in 1802. The five recognized degrees include:

1. Chevalier (Knight)
2. Officer (Officer)
3. Commandeur (Commander)
4. Grand Officer (Grand Officer)
5. Grand Croix (Grand Cross)

In the United States the Chevalier of the Legion of Honor is awarded in official ceremony through the French Embassy in Washington DC.   The Honor is awarded for excellent civil or military conduct delivered, upon official investigation.  The daily operation of the French Order is conducted under the authority of the Grand Chancery. 

This award strengthens global relationships and brings positive attention to the service organizations connected to the individual receiving the Honor.

Wisconsin Governor Walker Prepared For Delay With Federal Exchanges

On Thursday November 14, 2013 President Barack Obama announced the Health Insurance Companies could, at their discretion and with permission from State Insurance Commissioners, keep policies that had been scheduled for elimination for not meeting (ACA) Affordable Care Act standards.

Within minutes Wisconsin Governor Walker said he would delay for 3 months his plan to move 77, 500 Wisconsin residents out of State health programs and in to the online Federal Insurance Exchange.  Demonstrating a powerful response that immediately establishes relied on coverage for Wisconsin's needing population.  

Representative Joan Ballweg explained the State budget included provisions to continue Badgercare and (HIRSP) Health Insurance Risk Sharing Plan eligibility at current levels if Wisconsin experiences a delay in the implementation of the Federal Health Program. 

Governor Scott Walker will call lawmakers for a special legislative session in early December to pass the change to keep those 77,500 Wisconsin citizens from losing State Badgercare Plus coverage in January before the Federal Insurance Program is equipped to serve them.

Governor Walker will include a legislative proposal that would allow (HIRSP) Health Insurance Risk Sharing Plan to extend coverage for an additional 90 days.  This would not require HIRSP members to stay with the plan but if it is working, they could be served till March 31, 2014.   This demonstrates Governor Walker endorsing a broader range of options than the exchange currently makes available.

Some areas in the State of Wisconsin can benefit from an increase in the number of available coverage options.  Governor Walker has a proven history of working well with the Wisconsin Insurance Industry.  It is likely that the health insurance industry would continue existing plans with Governor Walkers encouragement.  Certainly we must applaud Governor Walkers fast, effective response to a very real problem facing many Wisconsin residents.   Home Care Path encourages readers to follow this issue as it unfolds in the coming months.

Time To Prescribe More Statins

Two research driven information sources The American College of Cardiology and The American Heart Association have fostered guidelines to expand the practice of prescribing statin drugs.   The purpose is to reduce the incidence of heart attack and stroke.

Statin drugs lower blood cholesterol levels by inhibiting HMG-COA Reductase.  HMG-COA Reductase is a liver enzyme used by the body to create cholesterol.  The statin lowers cholesterol by disrupting the enzymes  availability to participate in the assembly of cholesterol.

Once formed cholesterol increases plaque build up on artery walls which slows or stops blood flow.  Heart attack or stroke can occur with no blood flow.

So, why have statin drugs not become available to consumers over the counter?  Some medical providers are for allowing statins to be sold over the counter, and some are against the idea.   
Those against say elevated blood cholesterol levels do not generate symptoms that would prompt patients to make informed decisions about statin drug dosing or benefits.  Those for believe making the statin drugs available to consumers over the counter would dramatically lower the cost.

Reiki Energy and healing use

Reiki can be described as a non-physical healing energy made up of life force that is guided by higher intelligence , or spiritually guided life force energy.  In healing Reiki energy seems to have an intelligence all its own flowing where it is needed in the client and creating the healing conditions necessary for the individuals needs.

This energy is flowing through everything on the planet.  This energy flows around and through the individual's body all the time.  This unseen energy supports the physical organs and tissues as it flows through and about the body.  A disruption in the flow of this energy can contribute to an unhealthy condition that shows itself as a sickness in the body.

A persons mind (thoughts and feelings) can contribute to an effect in this field of energy circulating in and out of the body.  Often the Reiki practitioner can intuit knowledge of changes in the flow of this energy through sharpened senses. The Reiki practitioner can implement this non-invasive  healing technique to help correct the disruption of energy flow facilitating balance and bringing healing.

Levosimendan Going To Drive OXBT Shares Higher

Levosimendan is a cardiac medication proven to be effective in reducing mortality among patients with reduced heart function as they were having surgery.  Levosimendan is a calcium sensitizer that can be administered intravenously (IV) to patients with acute decompensated congestive heart failure (CHF).  The drug helps dilate vessels, increasing heart muscle contractility without increasing the oxygen consumption requirement.

Oxygen Biotherapeutics Inc. NASDAQ stock OXBT signed a definitive agreement in October 2013 for exclusive rights to develop and market Levosimendan in North America.  This transaction between OXBT and Phyxius Pharma Inc. is valued at approximately 4.8 million in stock.

FDA acts to improve medical providers access to needed drug therapies

The ability of the medical community to effectively treat a patient with a prescribed drug is dependent upon the available supply of the medication.  When a hospital or clinic do not have a medication for treatment it is called a drug shortage. The supply is depleted, causing patients in the
service area to go without the benefit the medication could have delivered.

The (FDA) Food and Drug Administration has proposed two new rules on October 31, 2013 to help prevent drug supply shortages.  Rule one involves implementing a strategic plan to coordinate an organized approach to reported drug shortages.  This means:

A. Immediately assess the drug manufacturers reported shortage

B. Immediately contact alternative drug manufacturers capable of filling the shortage

C. Speed up the process to certify a drug manufacturers site to help prevent an approaching shortage.

The second FDA proposed rule requires all drug manufacturers to notify the FDA of permanent discontinuance or a temporary interruption of manufacturing likely to disrupt their supply.  This is early reporting to head off a shortage.

Tips For Accompanying A Person To The Hospital

Having someone along when you enter the hospital can prove to be a big help.  Some things to consider include:

1. When you are sent for a test or scan where is the person who will interpret the results located?

2. When you go for the MRI how will the doctor's nurse know you have gotten your MRI?

3. Who is the lead hospital staff giving you an over view of the ongoing flow of the visit?

4. How can you be sure not to take the same diagnostic test twice?

5. Where are you stationed when the lead hospital staff is making calls and looking up information?

6. In the absence of the hospital lead how does the doctor know which patient you are?

7. How is the prescribing provider receiving the updated test results?

8. Once the prescribing provider updates the orders how are they coordinated?

These are some things to consider when observing the inpatient hospital process.  Knowing how departments (care transitions) connect can improve the quality of the persons hospital visit.

Twitter Constructs Glitch Free IPO on NYSE

As one of the most progressive collaborative activities Twitter organized a mock period of share trading on the weekend of October 26, 2013.  The NYSE conducted a full run with agents from registered firms simulating buying and selling.  A test run to observe the flow of Twitter share trading.

Twitter has authorized 70 million shares of the company at between 17.00 to 20.00 per share.  The (IPO)  Initial Public Offering goal is to raise 1.4 billion dollars.  November 7, 2013 marks the first day of real Twitter shares trading. 

Glitches can occur with the online transition process which can produce trading delays and order issues.   Glitches mean revenue loss.  Twitter teamed up with the (NYSE) New York Stock Exchange operators to perform a period of mock share trading.  The improvements post test run leave Twitter hoping investment firms report a streamlined trading experience.

Wisconsin Fish Fry Price So High

Why the heck is the Wisconsin fish fry costing so much?  Under Current News has boats reporting unusually slow fishing the latter part of 2013.  This translates to less cod and haddock available to meet the strong market demand. 

Recently haddock headed and gutted prices hit a 7 year high.  This is the highest cost has been since June of 2006.  Fishing sources theorize warmer ocean water temperatures are causing cod, haddock to swim closer to the surface (Russia, Norway side) making themselves harder to catch. 

Likely quotas pre set to protect the oceans fish beds will not be met for 2013.  This means no big cod or haddock accumulations for the hungry food services.  Thankfully Wisconsin still has the precious blue gill to fall back on.  Home Care Path encourages viewers to watch the Friday fish fry prices.

Out Of Pocket Limit 6350 Health Plan

Under Federal law out of pocket limits cannot exceed 6350.00 for an individual.  Once the out of pocket expense limit is reached your plan covers one hundred percent of the eligible expenses.  Your monthly premium does not count in the calculation of your out of pocket expense.

Home Care Path supports Wisconsin Senate Bill 101 Assembly Bill 151

Senate Bill 101 introduced by Senator Alberta Darling and Assembly Bill 151 introduced by Representative Pat Strachota create public policy designed to improve individual access to oral cancer treatment options.

Drug manufacturers are fast developing oral pills that can better target specific areas in the physical body preventing undesired (cancer) mass growth. The pills can mean decreased side effects (loss of hair) and improve efficacy (performance) while supporting quality of life. 

The difficulty is health insurance will pay more of the cost for a patient if they receive IV infusion (medical benefit)  of chemo medication cause the cost is perceived as being less. 
The health insurance industry categorizes the newly developed oral cancer treatment (pills taken orally) as a benefit of an individuals drug insurance plan.  The drug insurance provider places the chemo oral treatment pills in the most expensive tier if they offer it at all.

The IV infused chemo drugs can be much more labor intensive, requiring specially trained staff to infuse the medication.  The procedure involves needles and carries a risk of infection as well as additional pain and inconvenience for the patient.  Pills can beat out procedures in almost anyone's mind. 

Drug manufacturers understand this as evidenced by approximately 25 percent of the developing chemo treatment medications (400 on course now) for utilization with a prescription are now oral.
It seems a way for the insurance industry to add value to their service is to find a way to get these new effective oral medications in the current health care delivery system.

Physicians Plus Insurance Corp Expands in to Illinois and Iowa Health Markets

Physicians Plus Insurance Corporation is located 2650 Novation Parkway, Madison Wisconsin 53713.  Physicians Plus Insurance Corporation employs an integrated (managed care) care delivery model.

Health Insurance Products Saying Good Bye To Pre-Existing Condition

Pre-existing condition is a health condition that was diagnosed and or treated within six months prior to when the participant enrolled in the health insurance plan.  The pre-existing condition exclusion period was attached to most newly enrolled participants.  This means coverage could be excluded for a medical condition that existed prior to enrolling in the new health insurance plan.

After January 1, 2014 the pre-existing condition exclusion period will be a completely prohibited health insurance industry practice.  The health reform law prohibits the denial of coverage or charging higher premiums due to a pre-existing condition for plan and policy years beginning January 1, 2014.

Twitter IPO TWTR has a plan

Kathleen Falk will function as the key representative of HHS Secretary Kathleen Sebelius in collaborating with Federal, State, Military, Local, and Tribal representatives on Health and Social Services issues. 

This is a pivotal role in shaping the transition of an evolving health care delivery system to better serve larger populations experiencing a greater prevalence of symptoms of chronic disease conditions.  The work is to establish a unified health care delivery that better links pay source, medical intervention, and help to recover in the community.

Community Hospitals Brace Themselves With Roll Out

Community hospitals continually adjust their service to best  meet the local populations medical needs.  The name of the new game is cost reduction.  Hospitals are challenged to comply with Medicare cost containment regulations and stay a float during political delays with the health insurance exchange.

Expect to see tighter staffing of local health care facilities.  Health delivery systems will staff according to operation volumes. 

Discipline specific departments will be instructed to develop appropriate drug and lab formularies to reduce waste.  Eliminate expensive variation, train staff to a predictable system of utilization that aligns with best practice. 

Providers practice to the fullest extent of their license.  The doctor will not be employed to do the tasks a nurse can perform.  Skilled medical staff will not be employed to perform the tasks non-medical staff can perform.

Home Visits Shifting Community Perception

The State of Wisconsin has developed the IRIS program, the Family Care Program, and the Wisconsin Partnership Program, which serves nursing home level of care patients who choose to reside in the community.  This population receives home visits to maintain wellness. 

Local hospitals have placed health care services in the same geographical location and call themselves health care centers.  These providers now build large outpatient clinics to serve the area citizens.  Following an outpatient procedure many individuals benefit from the help of another (home visits) for a period of time.

A recent survey conducted by the Health Care Intelligence Network www.hin.com of New Jersey found that while 79 percent of the hospital health care system conduct home visits only 0 to 10 percent of the total population receive a home visit.  With a Federal, State, and Health Care Industry shift toward serving more individuals in the community the 0 to 10 percent reported figure should begin to climb.

Forty three percent of the respondents report post discharge as the primary reason for a home visit.  This survey involved 155 organizations.   The data indicates an individual is selected for a home visit by a process that includes chart reviews, electronic health care records condition program, hospital census, and self reports.

Insurance Exchange and Minimum Value Standard

Under the Affordable Care Act the employer sponsored health insurance plan sold to employees must meet a Minimum Value Standard.  An employer sponsored health plan meets the "minimum value standard" if the plans share of the total allowed benefit costs covered by the plan is no less than 60 percent of such costs (Section 36B (c)(2)(c)(ii) of the internal revenue code of 1986.  Health and Human Services rules must be utilized for determining minimum standard value.

Minimum value is calculated (HHS MV Calculator) by dividing the anticipated covered medical spending of essential health benefits (EHB) coverage (for the population covered by a typical self insured group plan) by the total anticipated allowed charges for EHB coverage for a typical self insured group health plan population. 

This Minimum Value Standard (actuarial value) measures the relative generosity of benefits covered by the health insurance plan.  Under the ACA Affordable Care Act a health insurance plans actuarial value (60% is minimum value standard) indicates the average share of medical spending that is paid by the plan, as opposed to being paid out of pocket by the consumer. 

So, we can see the actuarial values (60% is minimum value standard) are used to categorize insurance plans in to health benefit tiers.  See below:

Bronze = 60%  (this is the minimum value standard)

Silver = 70%

Gold = 80%

Platinum = 90%

Gold of course being more generous than bronze.  An employer group who purchases an insurance product through a work setting would need a plan that delivers a 60 percent contribution to the total share of medical spending.
