Family Care Program Expands In Wisconsin

Seven Counties in Wisconsin (Brown, Door, Kewaunee, Marinette, Menominee, Oconto, and Shawano) will have access to the State of Wisconsin administered Family Care Program.  This facilitates the operation of Care Management Organizations (CMO's) who staff multi county regions.  The Care Management Organizations contract with local service providers (www.homecarepath.com ) who can deliver the needed in home support.  The County operates the Aging and Disability Resource Center which intakes residents for assessment to determine eligibility for public funded programming.

Family Care Program participants are typically individuals who demonstrate a nursing home level of care on the (FES) Functional Eligibility Screen and possess assets below the listed limit.  The benefits specialist working through the (ADRC) Aging and Disability Resource Center performs the screening and then provides the individual and family with details about the varied programming.

The State of Wisconsin would prefer to have 2 or more (CMO's) Care Management Organizations functioning in the defined multi county region.  This allows the benefits specialist in the Aging and Disability Resource Center to give the individual and family multiple options to choose from.  The Care Management Organizations will employ a (Team) social worker, registered nurse, and unit clerk, to help you manage your Medicaid benefit.

The teams can be more responsive to an individuals change in condition and coordinate the services to fill the need.  This is a needs based approach.  This can be difficult for the community at first to understand because not every one receives the same services.  The person receives customized service  that can meet their individual need.  In this model the entre Family Care population is sharing the resources to serve more people.

Home Care Path is an in home supportive care service that contracts with Care Management Organizations to serve individuals who need help with meals, bathing, dressing, laundry, medication reminders, walking, scheduled toileting, transfers, memory loss, and so much more.  Home Care Path contracts with Family Care, Partnership, IRIS, Long Term Care Insurance, VA Non-service Related Pension, and private pay at 20.00 per hour to help elders in their home.

Did Ya Get Your Fishing License Reminder

The Recreational Boating and Fishing Foundation located at 500 Montgomery Street, Alexandria, VA 22314 is a nonprofit organization dedicated to growing participation in fishing, boating, and aquatic stewardship.

The Recreational Boating and Fishing Foundation forges partnerships with State agencies like the Wisconsin Department of Natural Resources.  This authorizes the Recreational Boating and Fishing Foundation to implement a fishing license marketing program aimed at increasing sales.

This comes in the form of a post card announcing it's time to renew your fishing license.  This is an attempt to link preservation and the sports through collaboration between the State of Wisconsin DNR and the national nonprofit Recreational Boating and Fishing Foundation.

Diagnosis of Hep C Not So Scarey With New Oral Meds

Hepatitis C is a virus (inflamed liver) spread when exposed to infected blood.  Recent drug studies published in the New England Journal of Medicine demonstrate major improvement in treating Hepatitis C.

Formulary combinations (Hep C cocktail) Simeprevir, Ledipasvir, and Sofosbuvir, to name a few can be delivered as an oral administration. Marketing oral drugs to consumers will take the complexity out of periods of medical treatment.

The oral consumption of Hep C cocktail has demonstrated sustained virologic response in recent trials.  Simply, the oral medications seem to work to reduce the presence of the Hepatitis C virus in the body.  This can facilitate the utilization of an on going monitoring process that can be conducted by the primary medical doctor in the community clinic setting.

The pharmaceutical industry is now challenged to produce a supply chain capable of distributing an affordable form of this very important medication.  This is clear progress in the treatment of the Hepatitis C virus.

PATH ALONG Transitional Consultant Inpatient Risk Of Infection

Current data indicates 5 to 10 percent of all patients nation wide are exposed to and develop an infection with a hospital or nursing home stay.  Wisconsin's PATH ALONG program coordinates an inpatient consultant who can assist seniors with a customized plan of care.

Technology today is testing a breath analyzer (Isomark Canary Breath Analyzer) where infection can be noted in the body up to 2 days before an elevated white blood cell count would display for inpatient lab staff.  That sneaky infection is brewing even before the traditional blood lab test can detect it.

Every individual discharging from a hospital or nursing home carries a risk of infection with them in to the home setting.  This can be especially problematic for frail elders who just desperately want to be home.

An infection can cause some inpatient discharges to revert from the in home recovery stage back to the acute need for rescue stage .  PATH ALONG consultants work with the elder to determine where the chronic event resides on the health continuum.  This can bring the most likely expression of acute symptoms to light.

The connection to proper treatment is strengthened by defining the primary care rapid path.  This is an evidence based approach that fulfills the care assurance role.  This is an imperative  piece of delivering a measure of prevention while improving the seniors quality of life.  Some elders transitioning from a hospital or nursing home to a community dwelling can benefit from the service a PATH ALONG Transitional Consultant can deliver.

Wisconsin Wants Prescriber Enrolled In MA

Wisconsin seniors being served by Department of Health Services programming (Family Care, Seniorcare, Partnership) will need to have their prescriptions written by MA certified providers enrolled in the Wisconsin MA (Medical Assistance) programming.

A Wisconsin pharmacy may not be able to fill your prescription if the prescribing provider is not enrolled in Wisconsin Medicaid.  This is because the Wisconsin MA fund will not reimburse the local pharmacy claim for filling a prescription written by a physician who is not enrolled in Wisconsin MA.

This is a mandate listed in the Accountable Care Act within Medicaid code of federal regulation 42 CFR 455-410 reads The State Medicaid Agency must require all ordering or referring physicians or other professionals providing service under the State MA plan or under an MA waiver of the plan to be enrolled as a participating provider.

This demonstrates the reach of  Federal control through policy over State implementation of the health service programming.  The local pharmacy needs an MA enrolled prescriber to be reimbursed for medications dispensed through some Department of Health Services Programming.

Wisconsin's PATH ALONG Data matches National Values

PATH ALONG staff reviewers proclaim the microcosm (PATH ALONG) data matches the macrocosm (Centers for Medicare and Medicaid) hospital readmission values.  The Centers for Medicare and Medicaid  (CMS) reports one in four patients being served in a skilled nursing facility are readmitted to the hospital within 30 days.

The Wisconsin PATH ALONG model data indicates individuals being transitioned from a nursing home to the community carry a huge risk for hospital readmission.  A structured data review by PATH ALONG researchers indicate 22 percent of individuals being transferred from a nursing home to the community resulted in a hospital readmission.

This is significant when one considers Wisconsin State Programs like  Family Care, Wisconsin Partnership, and I Respect I Self Direct ,  are specifically designed health  initiatives  to support individuals in their own homes.  The Centers for Medicare and Medicaid (CMS) report two thirds of individuals being served by a nursing home are enrolled in Medicaid.

Simply stated, individuals being transitioned from a nursing home to the community carry a huge risk of hospital readmission.  The elevated risk does not disappear with nursing home discharge, but actually follows the patient into the community dwelling.   Structuring supportive home care to improve health outcomes for an individual transitioning from a nursing home to the community requires advanced care planning.  PATH  ALONG staff recognize the need to insert strategies implemented to improve the process for addressing acute changes of condition.

PATH ALONG model staff have adapted the following initiatives in to their practice.
  • define path acute condition changes are treated
  • define primary care rapid path
  • define communication path per discipline
  • define care transitions path
  • define SNF path for performing ADL's
  • define navigation path between multiple pay sources
  • define additional resources secondary to acute changes in condition
  • define path for sharing information
An individual leaving a nursing home for a community dwelling can expect the high risk for a hospital readmission to follow them.  Expect acute conditions presenting in the community dwelling to require additional resources.  Expect the individual to require rapid access to advanced primary care.   Indviduals expecting to transition from a nursing home to the community will need more from all involved.

Home Care Path Wisconsin Analytics

Home Care Path www.homecarepath.com is a leading senior care provider serving the south central Wisconsin area since 2010.  Home Care Path data review goes well beyond the categories of expense, profit, and frequency of use.

A big part of data review is fraud prevention.  The effort of Home Care Path involves both payment prevention and reduce the risk of collusion across the multiple participating programs. This prevents the need for an organization to recover monies already paid.

That being said, the cost of gas and difficulty of the work performed by Home Care Path staff the organization believes that staff who come and help the senior for the time recorded they should be paid.

The evolution of analytics as relates to the real time operation of Home Care Path can be perceived as an advanced model of risk reduction.  Participating in a review process that encompasses variation in focus to maintain senior care industry stability amidst a lot of health care system change.

Social Network Analysis - notes patterns of ties
Pattern Recognition - most likely matching
Data Mining - patterns within collected operational information
Predictive Modeling - reveal patterns that point to opportunity
Anomaly Detection - not conforming to expected pattern
Network Analytics - ties roles to activity

It is important to understand all the individual information utilized with the operation of Home Care Path remains confidential.  The review process does not link an individual to the pieces of data. Privacy and confidentiality are observed in the strictest sense.

An immediate correlation has been documented between availability (not present for scheduled visits)  and movement (frequent switching between service providers).  The category of movers tend to always be movers.  So, if one tracked the individuals moving between service providers over time you would reveal the same people.  Movers should be targeted for participating in new options.  The population with a propensity to move would be most likely to enter in to new programming.

Collaborative Page To Reduce Hospital Readmissions

Information technology is being challenged to establish a software application capable of producing an immediate computer screen formed from recorded health care system contacts through out the varied medical providers locations.  This would be a collaborative page of an individuals medical chart.

This would give an assessing medical provider the ability to compare assessments against past diagnosis, previous tests, prescriptions, and historical record.  This is real time, community wide, immediate review of an individuals health data. New York's Health Information Exchange is an example of developing and utilizing this type of technological application.

The collaborative page is a technological advancement that could improve the quality of a medical providers assessment process.  This is an essential piece of the hospital providers responsibility to determine the need for an inpatient stay.

The collaborative page facilitates digital communication that can be viewed any time through a pass word protected , cloud based portal , that can comply with existing HIPAA privacy laws.  Home Care Path staff expect to see a technological solution designed to unify immediate access to all medical information recorded with past provider visits.

Once the technology has proven its ability to deliver a collaborative page expect State governments to shape medical industry regulations to best adapt this needed add on.  This is a means to manage hospital readmission rates while adding a measure of care assurance to the providers assessment process.
