2 Things U Must Know About SSM Dean OpenNotes

Sisters of Saint Mary Dean Health System has scheduled the implementation of OpenNotes.  This will give patients access to Doctor Notes after a Medical Clinic visit through a smart phone. Many elders do not have the technological devices or computer experience needed to read their own doctor visit notes.

So we are looking at 2 major conceptualizations when we think of the OpenNotes effort.

1. Supportive home care bridges the doctor notes to the elder

2. The bridging activity links medical provision to non-medical supportive home care

Supportive home care is the bridge connecting the technologically challenged senior to the now available medical doctor visit notes.  Senior support partners will be the ones using their smart phone to help the elder look up the post visit doctor note.

This bridging activity links medical provision with non-medical supportive home care.  Post visit technological review of the doctors note will count for engagement of the patient which is a quality measure.  An attached technological program will count the patient that logs in and reads the post visit notes.  Favorable numbers of patients performing a post visit technological review will improve reimbursement through public funded insurance plans like Medicare and Medicaid.
