Wisconsin Dementia Care Specialist Program and Reducing Readmissions

Wisconsin Law Makers are crafting bills that authorize the Department of Health Services to strengthen access to Dementia Care Services through County Aging and Disability Resource Centers.  This is an essential piece of bolstering safety with community hospital efforts to reduce readmissions. 

The goal of the Dementia Care Specialist program is to support the ability of individuals diagnosed with memory loss to remain in their homes longer.  The familiar setting improves quality of life, while preserving precious resources.

Working from the Aging and Disability Resource Center the Dementia Care Specialist supports staff across the multiple programs being delivered in the community.  A component of this program includes organizing functions that bring private and public providers to collaborative efforts aimed at creating a more dementia friendly community.

The Wisconsin Dementia Care Specialist Program is instrumental in facilitating the non-medical provision of memory loss support to assist existing medical (hospital- nursing home) delivery to operate in a safe productive manner.  Families caring for individuals with dementia demonstrate need for assistance with tasks that do not always align with the standard medical delivery occurring across Wisconsin communities. 

Families caring for an individual manifesting undesired symptoms of dementia and confronted with a problem that cannot be resolved will gravitate toward the familiar hospital emergency room setting for help.  The Wisconsin Dementia Care Specialist Program is expected to foster the development of services that better meet this crisis type need. 

A Dementia Friendly Wisconsin will involve private partners capable of delivering innovative non- pharmacological supports that exist to improve the ability of families to keep their loved ones experiencing symptoms of dementia in the home longer.
