Track Memory 2 Zero an original Alzheimer's Dementia Care Coordinator Model


                       TRACK  MEMORY  2  ZERO


Track Memory 2 Zero is a 10 level progressive schedule used by care coordinators to understand increments of memory loss related to a diagnosis of Alzheimer's Dementia Disease.   With memory loss:

-         The elder will stop bathing because they do not know how to work the shower long before they are physically functionally unable to bathe.

-         This is because the IADL’s (more complex) fall away before the ADL’s

-         By medical design long term care insurance and public programs do not participate in the care until the later need for functional help with ADL’s can be assessed

-         Change in payment frequency due to memory deficit adversely effects the seniors credit report which is utilized during the assisted living intake process

-         The factors that inhibit the documentation of a medical diagnosis are numerous

-         Without a medical diagnosis the provider is unable to properly prescribe medications needed to manage undesired behaviors

-         The crisis that drive transitions do not display needs an individual discipline can effectively meet on their own

-         The increased incidence of Alzheimer’s Dementia and manifestation of undesired symptoms is better served when viewed as a community wide issue
