Wisconsin Platform To Boost Collaboration For Families and Caregivers

Home Care Path is working to update its basic architecture to a more modern web native platform to facilitate the interaction of families and elder care givers.  This strengthens the capacity to improve the essential  core service of collaboration.

The platform is scheduled for implementation in the second quarter of 2015.  Designed to be more than a receiver for medical providers trying to fulfill their 2nd stage meaningful use requirement by transmitting medical information to a 3rd party. 

The platform will be a functioning network facilitating communication of families, care givers, and active providers across the rural Wisconsin landscape.  The launch will be timely in that the approaching changes in the Wisconsin budget will make collaboration an essential service.

How families dependent upon public funds as a pay source for in home services will comply with adjusting requirements will be the priority.  Sharing information to reduce fragmentation and firm up stability of  Wisconsin house holds with long term care needs. 

Home Care Path is employing a proactive approach to assist local communities who will soon be confronted with rapid change.  We are all in this together.
