Re-structuring Service 2 Accommodate Wisconsin Budget LTC Changes

Home Care Path is proactively adjusting service to help Wisconsin communities manage the changes to long term care with the approaching 2015 budget.  Wisconsin budget initiatives designed to improve outcomes while containing cost can be perceived as disruptive and abrupt.

Early July 2015 could begin the privatization of Aging Disability Resource Center tasks and replacing Care Management Organizations with insurance plan providers. This is a broad change across a vulnerable population  with interdependent community services sharing significant risk.

Home Care Path has identified this period of transition as an immeasurable elevation in the risk for hospital readmissions.  Families perceiving trouble immediately attach to the known.  The known in this approaching service shift is the community hospital.

Home Care Path employs the PATHALONG https://homecarepath-public.sharepoint.com/pathalongmodel model to help communities reduce hospital readmissions.
This is a strong care transition communication tool.  This connects families to information capable of assisting with the important process of coordinating needed service.

The core service being delivered through the Home Care Path upgrade is collaboration.  This is an essential piece of the Long Term Care platform re-structure.  This is a unique approach to leveraging digital technology to strengthen Wisconsin families over all quality of life as the population ages.
