The Commission on Long Term Care Report is Published

The Commission on Long Term Care which was formed June 10, 2013 under a law signed January 2, 2013 has published a final report http://www.ltccommission.senate.gov/Commission%20on%20Long-Term%20Care%20-%20Final%20Report%20-%209-18-13.pdf.  The final report is required by Section 643 of the American Taxpayer Relief Act of 2012 (P.L.112-240). 

The Commission on Long Term Care was formed to examine long term care services and supports in areas of provision, staffing, and viable pay source.  This period of time is unique in that the largest generation in history is growing old. The data indicates over 12 million Americans of all ages need long term care supportive service.  This need will grow dramatically over the next 2 decades.

Financially the only way to make an insurance product that can effectively serve this population is to spread the risk over a huge base of plan holders.  The sustainable structure for this insurance product is currently not available.

The Commission describes the future of Long Term Care Services and Supports as being integrated with public programming and the medical providers.  The Commission indicates community based supportive care service providers ( www.homecarepath.com) will need to participate in outcomes focused care management.  Home Care Path staff discuss this approach as the shift from selling hours to developing a safe plan that meets the individuals needs.

Home Care Path as a Wisconsin long term supportive care provider develops a plan of care with the consumer, caregiver, family, and medical staff in mind.  This inclusion improves participation in person and family centered care.   The ongoing updates delivered to Home Care Path staff and family through newsletters, web site, blog, App all improve access to caregiver information.

Home Care Path staff receive ongoing training, which includes a manual (written updates) and in home training, with additional help as conditions change.  Home Care Path flexible care plans (inclusion of family) and no scheduling minimum enhance family and care giver engagement.  Including the medical providers in the care plan strengthens integration and allows families the opportunity to deduct the cost of the services from taxes in some situations.

The Wisconsin PATH ALONG model includes staff trained to provide supportive care services for an individual throughout the variation and physical locations of the medical delivery offices, clinics and hospitals.  As the medical system updates the community facilities to provide treatment to an aging population with chronic disease processes PATH ALONG staff can attend to the supportive care needs. 

Home Care Path and the Wisconsin PATH ALONG model work with available Federal, State, and Military benefit specialists to establish a service that provides the person and family an opportunity to maximize utilization of the available public programs.  This can involve facilitating access to participating in organization of the data required to maintain service.  The Commission on Long Term Care Final Report is published and the work continues.
