Hospice and Skilled Medical Home Care Social Workers Resource

Skilled medical home care and hospice are typically reimbursed through the Medicare fund which contains regulations that define benefit limits.  Both skilled medical  home care and hospice require a doctors order, and the person to have a Medicare card. 

The doctor writes an order indicating the beneficiary meets the medical eligibility requirements to be served by the program.  This essentially means the elder needs skilled medical workers to help them in the home. 

Some seniors will be served by both these programs and suddenly their needs change and there is not a level of care that requires a skilled medical staff to perform this service.  At this point the social worker in the skilled medical home care or hospice program must go to the home and discharge the patient.
