Home Care Path is constructing a dementia care platform .  The initial plan defines 3 primary implementations to create the network effects that will make the platform valuable. 

- Attract will draw elder support partners and families to the platform to participate in the exchange

- Enable will supply the information and applications to make the mutual exchange easy

- Connect will define a process for hiring elder support partners who can come and help

Families can view the elder support partners listing online.  The family and elder support partners close by can talk on the phone - discuss the senior family members needs.  The family can run a back ground check and schedule a face to face visit.  The elder support partner can come to help on pre-scheduled periods of time.

This is a non-medical service and can be understood as support for the early dementia phase manifestation of needs under the category called Instrumental Activities of Daily Living (IADL's).  This is an essential service which fosters early engagement, prevents crisis like dementia events, and strengthens the families ability to keep their loved one in the home longer.
