Wisconsins PATH ALONG Receives Call From CMS

Wisconsin's PATH ALONG Model had submitted form OMB No. 0938-1124 Community Based Care Transitions Program Applicant Data Sheet in January 2013.  The applicant data sheet was accompanied by a proposal to deliver a comprehensive approach to readmissions management for a south central Wisconsin hospital setting. 

Wisconsin's PATH ALONG is an original model that develops structure in four segments of service delivery to help reduce readmission rates.

1. PATH ALONG staff participate in strategies to manage the repetitive ER visit loop.

2. PATH ALONG staff participate in strategies to manage the use of observational outpatient status stays.

3. PATH ALONG staff accompany the consumer on the inpatient stay utilizing copyright content to improve quality and assure a safe recovery.

4. PATH ALONG staff adhere to an open needs based approach that connects supportive care when skilled care is not necessary.

PATH ALONG delivers an effective service which gives Wisconsin inpatient providers a quality referral source as response to federal regulations that reduce reimbursement rates for hospital readmissions.

On Wednesday, February 20, 2013 PATH ALONG received a call from CMS program administrator Juliana Tiongson who reports acceptance of recently submitted Community Based Care Transitions Program Applicant Data Sheets are on hold status for at least the next six months or more.

PATH ALONG is a privately paid service billed at 20.00 per hour.  PATH ALONG helps seniors during the hospital stay and outpatient treatment visits.   Staff accompany the consumer through the process.
