Wisconsin's PATH ALONG model helps seniors with the rapid ongoing transmission of instructions during a hospital stay.  The PATH ALONG model delivers a familiar structure to the process, helping seniors participate in post hospital care.  PATH ALONG provides needed support at the inpatient point of service which enhances the overall quality of the hospital stay.
