PATH ALONG Revolving Door Syndrome Dartmouth

The Dartmouth Readmissions Report is out.  The Readmissions Report is an ongoing project through The Dartmouth Institute For Health Policy and Clinical Practice located 35 Centerra Parkway, Lebannon, NH 03766. 

This report separated readmissions by two characteristics.

1. First admission involved surgery

2. First admission involved a (CHF) medical condition

Current values indicate an increased awareness that readmissions (Revolving Door Syndrome) are a costly undesired aspect of todays health care delivery.  Long time community health care planners have recognized that part of the problem is a lack of understanding by the user population.

The Dartmouth Readmissions Report demonstrates that higher readmission rates correlate with higher admission rates.  So the greater the hospitals user population the greater the chance for an elevated readmission rate. 

The study involved an interview process that seemed to indicate that severe illness and fatigue associated with a hospitalization reduced the patient's ability to retain important information.

The scant progress seems to indicate the solution to the revolving door syndrome involves a broader acceptance of community service providers.  Wisconsin's PATH ALONG model helps seniors during the hospital stay and outpatient treatment visits.
