Wisconsin Predicts Increased ER- Urgent Care Utilization

Wisconsin will be implementing change in to the family care program with the 2015 budget.  The population using the family care program is comprised of individual's at nursing home level of care who choose to reside in the community.

Chronic progressive disease processes contribute to the higher level of risk providers experience caring for this population.  This has meaning for inpatient providers shifting to a value based delivery model.  Hospital and nursing homes are incentivized to reduce the length of stay and prevent readmissions. 

The population eligible for the family care program demonstrate a greater frequency of meaningful deterioration requiring a hospitalization.  The deterioration periods of time do not tend to coincide with an inpatient care managers monthly wellness calls.

The time of transition from the current family care program to the new family care program is a period of elevated risk.  Families dependent upon scale personal supportive care- scale defines average time per task and average hourly rate may experience provision adjustments. 

This will have an adverse effect on shared savings performance benchmarks across participating accountable care organizations.  In times of extreme change expect families to gravitate toward the known - the familiar community hospital emergency room.
