The Patient Protection and Affordability Care Act H.R. 3590 was signed by President Barack Obama in March 2010. There are specific parts of the bill that address discharge planning. H.R. 3590 section 2717 describes a process for monitoring health outcomes directly related to the discharge process. Section 3025 describes financial incentives to encourage hospitals to reduce readmissions through appropriate discharge planning. House H.R. 3962 section 1151 describes fiscal mandates targetting the reduction of preventable hospital readmissions secondary to the discharge planning process.
