Home Care Path operates evidence based post discharge programming to support needs related to a patients recent inpatient stay and assist individuals to transition from a hospital or nursing home to their primary care clinical team.  This community approach makes it clear the care delivery is not to replace primary clinical care post discharge.

Basin is most often used by the patient with the PATHALONG model.

Staff can go right with the patient and help with difficulties that arise during the stay.  PATHALONG program staffing rates are 20.00 per hour.  More and more elective (scheduled) surgical interventions require the patient to have another to help.
