PC-bin Wisconsin's Independent Personal Care Assessment Service

Wisconsin 2015 budget legislates the Family Care Program from a Managed Care Organization to a Health Insurance Plan model.  PC-bin is an independent personal care assessment service developed to bridge the gap between the Health Insurance Plan and the local Personal Care Provider.

The Health Insurance Plan refers the Family Care beneficiary to PC-bin authorizing a registered nurse to conduct a face to face assessment. 

PC-bin forwards the ADL and IADL summary to the Primary Medical Doctor who completes the Insurance plan certification.

The Medical Doctor returns the certification documentation to the referring Health Insurance Plan. 

The Health Insurance Plan sends an authorization to the contracted Personal Care Provider who staffs the needed hours. 

Typically the Insurance Plan provides the required forms to the beneficiary who is instructed to have their Medical Doctor properly fill in and return.  PC-bin is a separate process that would serve the  individual's populating the new Family Care Program .

The Insurance Plan refers to PC-bin who assesses the beneficiaries personal care need which helps the Primary MD complete the certification for the local Personal Care Provider to receive the authorization to deliver service.
