Seniors Want Google Driverless Car

The media is all a buzz about the successful initial trips with the Google Driverless Car Prototype.  Wisconsin seniors are telling Home Care Path (www.homecarepath.com) staff how excited they are to have this being developed and made available. 

One elder stated this will flatten out those monthly insurance premiums.  There is no driver, so similar vehicles operated in similar areas will have the same low insurance rate.  This will take the insurance industry some time to adjust their service to best meet the consumers changing transportation need.

Seniors expect this vehicle to completely eliminate the cost of traffic fines.  The car is driverless, no driver to ticket.  This presents a huge change in the revenue stream many law enforcement agencies have become so accustomed to receiving.  Loss of traffic fines leaves City's struggling to find money for an operating budget. 

Senior's expect the State to have to come up with an alternative form of identification.  This would eliminate the need for a drivers license.  There is no driver to license. 

This is so perfect for seniors who have changes in cognition and physical function which results in a loss of a valid drivers license.  Wisconsin is a rural landscape full of transportation challenges.  This would mean precious independence for an elder living a bit on the edge of a city or town.

Nexium the purple pill goes over the counter

The (FDA) Food and Drug Administration has approved the over the counter Nexium 24HR.  Nexium (the purple pill) known chemically as esomeprazole 20mg goes (OTC) over the counter - no doctor prescription required.

Is Fall Prevention Working?

Fall prevention is one of the most studied, developed, and marketed products in the health care arena.  Constantly being updated to best manage the populations risk for an injury or death from a fall.

The health care industry no longer uses restraints to prevent falls.  Fall prevention became a JCAHO mandate in 2005.  Flyers marketing live workshops on fall prevention are distributed to providers multiple times per year. 

Fall prevention for the frail elderly has been big business for the purveyors of technology and equipment tools.  Yet, the CDC has reported the death rate from falls among older men and woman has risen sharply over the (August 2013 WISQARS) last decade. 

This would seem to indicate the mass distribution of equipment and delivery of instruction has not stabilized or reduced the death rate from falls among older men and woman.  We may need a measure of effectiveness?

Should Hospital receive more than MD Office for same service

Legislative representatives continue to struggle with health care delivery and Medicare reimbursement.  Specifically, should Medicare be paying providers the same for a service, no matter where it is performed?  Why would the patient care?

As the Medicare reimbursement goes up so does the patient's out of pocket contribution.  A similar treatment regimen for cancer can be conducted in a hospital outpatient delivery setting or a physicians clinic office.  Same treatments, but the hospital receives a much higher reimbursement rate.

Suddenly a large hospital network purchases the physicians clinic office practice and the existing population must pay more for the same service.  Medicare is a pool of money that public representatives will continue to develop strategies to reduce out flow.  Introducing bills that would require Medicare to pay providers the same for a service no matter where it is performed is one of those strategies.

Not Every Patient Takes Their Medications

Home Care Path staff have long understood patients do not all take their medications as provider ordered.  The focus is more to ascertain what the patient is taking as opposed to making someone do something they do not want to do.  This action helps manage the increased risk created for the prescriber's measure of liability with attempts at pharmacological treatment.

This became evident with the Tomaszewski and all study titled adherence to antihypertensive (high blood pressure) drug treatment recently published in the British Medical Journal.  The study demonstrated about 25 percent of the participants were not taking medications as provider ordered.  Adherence was measured using high performance liquid chromatography tandem mass spectrometry (HP LC MS/MS) urine analysis.  Despite being involved with a tightly controlled environment of an organized research project 25 percent of the participants did not take their medications as provider ordered.

CDC & WHO Confirm Middle East Respiratory Syndrome MERS CoV in U. S.

The Centers for Disease Control and Prevention (CDC) and the World Health Organization (WHO) have confirmed the presence of the MERS CoV virus in the United States.   MERS CoV was first reported on 2012 in Saudia Arabia.  The first confirmed incidence of MERS CoV in the United States is on May 2, 2014.  The second confirmed incidence of MERS CoV in the United States is on May 11, 2014. 

MERS CoV is a newer human infection, which means the body will not have built up immunity.  MERS Cov Middle East Respiratory Syndrome is a virus, meaning a microorganism that needs a host cell to reproduce.  Categorized as part of the coronavirus family known for crown like spikes on the surface.

The Centers for Disease Control (CDC) and the World Health Organization (WHO) are unsure where the virus came from (suspects the virus evolved to jump from animal to human) or exactly how it spreads.  The symptoms include cough, fever, and shortness of breath.

Basin Medical Clinic Coverage Verses Coordination

The community clinic is challenged to have a provider available when an elder must have a primary medical need met.  Changes in community clinic ownership can contribute to changes in staff structure.  Vacations, illness, and retiring providers can all result in medical visits being performed by different providers. This is coverage for care delivery which is different from coordination.

Coordination would involve a senior needing an MD order for a service (long term care insurance) or a piece of adaptive equipment.  You discover the elder's listed  primary provider is a (PA) Physician's Assistant who has not seen the elder in over 8 months .  The PA is actually on maternity leave with this request for an MD order.  The elder has seen two different MD's in the last 6 months, one visit resulted in a physical therapy evaluation and a walker with outpatient sessions.

Each clinic MD has a different nurse (the contact person) and the clinic has an internal policy that encourages the nurse to instruct the elder to come in for another clinic visit to receive the MD order.

Were the (PA) physicians assistant available their signature on the order could work if:
    - the PA is State certified
    - the PA has their own (NPI) National Provider Identification
    - the PA is in compliance with section 1861 (aa) of the social security act

Some payers will require the medical doctor to sign off for a face to face visit conducted by a physicians assistant, a nurse practitioner, or a clinical nurse specialist.  The medical doctor will have a documentation requirement which can be reimbursed using the G billing code (G0454).

Lucigen Scores 500k Wisconsin Economic Development Loan

Lucigen of Middleton Wisconsin has received a 500k Wisconsin Economic Development Loan.  This is to create rapid diagnostics for the medical setting.  Rapid diagnostics means immediate treatment and faster healing.
