Family Care Program To Expand In To North East Wisconsin

On Wednesday November 12, 2014 The Department of Health Services announced the Wisconsin Legislatures Joint Committee on Finance approved Family Care contracts for the Northeast Region. 
Two (MCO) Managed Care Organizations: Care Wisconsin and Lake Land Care District have been awarded State contracts.

Family Care serves individuals who financially qualify for Medical Assistance, and per (FES) functional eligibility screen meet a nursing home level of care, while choosing to receive services in a community dwelling.  The (MCO) Managed Care Organizations contract with community health providers  (Home Care Path https://homecarepath.sharepoint.com/Pages/default.aspx ) who deliver personal- supportive care services to Family Care Program members. 

This expansion will improve program access to individuals residing in Brown, Door, Kewaunee, Marinette, Menomonee, Oconto, and Shawno Counties.  Home Care Path would like to take this opportunity to wish the contracted MCO's much success in their effort to expand service across the Northeast Region of Wisconsin.
