Sauk County Care Transition Coalition

Home Care Path participated in the Sauk County Care Transition Coalition.  This is a local response to Federal (CMS) policy reducing reimbursement for hospitalization when a readmission occurs in less than 30 days.  The Centers for Medicare and Medicaid have forged policy that encourages hospital's to focus on discharge planning to prevent costly readmissions.

The Sauk County Care Transition Coalition bring together a diverse group of organizations who function to enhance the health and well being of the community.  This is a call to modernize collaboration pushing the independent practise in a bubble strategy toward an interdependent cooperative approach.   This calls for a frequency of communication that tightens connections beyond the simple referral process.

This verbalizes the notion that many area providers will serve an individual person at the same time.   Multiple providers participating in the care of one individual.   This meeting gives some shape to improving discharge planning and it comes in the form of cooperation. 

Home Care Path (www.homecarepath.com) noted the expansion of the service menu was directly aimed at reducing hospital readmissions.  Specific services include:

-inpatient advocacy follows the senior through hospitalization, rehab setting and back at home.

-crisis intervention with transitional counseling stabilizes an undesired condition that has resulted in repetitive trips to the emergency room.

-temporary help with a scheduled day surgery helps assure the senior is able to participate in the discharge instructions and be safe.

This is an exciting time for area organizations to learn about the services available to the Sauk County population.  Hoping for some benefit to both the individual and the community as a whole.  Home Care Path  stands ready to cooperate, participate and collaborate.
