Federal and State Want You Out Of Hospital and Nursing Home

The Federal Government and the State of Wisconsin are using law to steer policy toward serving seniors in their homes when ever possible.  As you read these words the infrastructure is being assembled to keep seniors out of the hospital and nursing home setting as long as possible. 

FEDERAL LAW: Patient Protection and Affordable Care Act

STATE RULING: Community First Choice Option

REASON: Studies show home and community based care can lead to better outcomes

HEALTH AND HUMAN SERVICES PROGRAM: Independence at home demonstration

EXPLANATION OF PROGRAM:  CMS (Centers for Medicare and Medicaid Services)  Innovation Center and Independence at home demonstration program work with chosen primary care practices to provide home based care to chronically ill Medicare patients.  Treatments are recorded and outcomes are monitored to test the over all effectiveness.

WHAT IS THE INCENTIVE: The State participating in the Independence at home demonstration program receives a six percent point increase in Federal Medicaid matching funds.

FOR SURE: Many individuals with long term chronic care needs are more likely to participate and follow through with prescribed treatment plans when care can occur right in their own home, no inconvenience of hospitalization or lengthy nursing home stay.
