Technology Finer Granularity Sleep Apnea CPAP

Public health (Medicare) funds embrace technology that delivers finer granularity with collecting and storing individual patient data.  Sleep apnea can be treated with wearing a (CPAP) continuous positive air pressure delivery with night rest. 

Once diagnosed, and fitted with a CPAP, the medical provider instructs the patient to utilize the CPAP for at least 4 hours per night.  Attaching the CPAP to an electronic digital application gives the clinic staff remote access to utilization information. 

The provider now knows if the patient is wearing the CPAP for the prescribed 4 hours per night regardless of the location the rest is occurring.   The risk of noncompliance (using CPAP less than 4 hour per night) involves the loss of public money (Medicare) as the pay source. 

Utilization managers will review individual data and refuse to allocate funds from Medicare for up keep, supply, and maintenance of equipment not being used as provider ordered. This tightens the relationship between patient, payer, and provider.  Risk and responsibility are being moved in to a more shared space.

Ebola Attacks The Vascular System

Ebola is a (Filoviridae) virus that appears like a long tube with a looped end under magnification.  Ebola is spread by contact transmission.  The precise natural reservoir is unknown.  Ebola progresses rapidly in humans which can result in some incidences of the actual diagnosis occurring after death.

First signs can include fever, headache, general pain, feeling tired.  Initial signs resemble the symptoms that manifest with many different bugs looking much like a case of flu.  Focus of the viral process is connected to the liver and the blood vessel endothelium.  Components of Ebola seem to attach to macrophage which then disrupt the lining of the blood vessels. 

Disrupting the integrity of the blood vessels allows fluid (blood) carrying much needed oxygen and cell nutrients to leak out.  This progresses to internal and external bleeding causing organs to lose oxygen and nutrients and fail.

Early on lab test antigen- capture ELISA can detect the presence of antigen.  Treatment involves quarantine and management of symptoms.  Currently each diagnosis of the Ebola virus is applied as a predictive application for the manifestation of the risk.  This is tracking with comprehensive interventions to contain the virus and prevent further spread.

NASDAQ Fascinated With Rise of FB Facebook stock share price

Facebook (FB) has captured the attention of NASDAQ stock market with climb in share price from initial IPO  38.00 per share up to today's 75.19 share price.  Share holders appreciate a stock that is growing. 

Those with a crystal ball anticipate Facebook adapting payment modes to embedded applications.  This blends popular communication, secure payment, with targeted marketing, on an electronic digital field of well visited community location.

Tammy Baldwin Legislation To Reduce VA Patient Wait Time

Wisconsin Senator Tammy Baldwin has introduced legislation Veterans Affairs Health Work Force Enhancement Act as a long term solution to address the unacceptable lengthy patient wait times. This legislation compensates for the under lying factors contributing to the long wait times.

The legislation forges policy designed to structure regulations that support the changes needed to more effectively serve the Veteran population needing medical care.

- mandate immediate staff shortage reporting to facilitate location specific additional medical staff residency allocation

- increase and develop the number of VA graduate medical education positions

- establish process designed to prioritize needed specialty training and strengthen internal retention

This legislation cues higher educational (medical colleges)  communities to develop and operate the medical instructional programs that can meet the identified staff training needs.  Technology providers can begin to install predictive analysis applications to improve needs identification aimed at managing through put time periods.  Senator Tammy Baldwin clearly communicates our veteran's deserve better with her timely introduction of this important piece of legislation.

Study Puts Schizophrenia On Solid Scientific Ground

Nature the International Weekly Journal of Science has published findings from research Biological Insights From 108 Schizophrenia Associated Genetic Loci.  This study links Schizophrenia to 108 new regions on the Genome. 

Identifying the new regions within the humans complete set of DNA delivers a biological approach to crafting an effective treatment for the undesired symptoms of schizophrenia.  This model of study provides evidence of the biological basis for a diagnosis of schizophrenia.

Clusters of genes that form patterns suddenly become relevant as a measure of risk for development of symptoms of schizophrenia.  With the genetic link confirmed research constructs step in to a scientifically embraced model of treatment development.

This can translate to a stronger support in the form of public money for much needed research.  The genetic links lend a measure of credibility to the ongoing tasks of study for the purpose of developing effective treatments.  This pushes the process beyond the minimum into a race toward a reachable goal.

Does Wisconsin Even Have A Doctors Office

The larger health care provider groups (hospital systems) have improved the structure of their care delivery in the face of payer (public and private insurance) reimbursement changes.   Payers have tightened the benefit by progressively reducing reimbursement amounts with hospital readmissions.  Payers have increasingly contracted with hospital systems to perform outpatient procedures rather than over night hospital stays. 

To manage change smaller rural hospital and clinic providers have sold their network of service to larger health care systems with a multi state delivery platform.  When a large health care system assumes operational authority the smaller provider network is transformed to mesh with the huge multi state delivery platform.

This new model of serving the community can include a few noticeable changes. 
1. Tighter inventory control
2. Rapid hospital path
3. Equipment upgrades with the expectation of performing more invasive interventions
4. Exemption from property taxes for non-profit under 70.11 (4m)

The buildings maintained by the larger health care system enclose more physical space and expand the menu of available services under an individual location.  Longer hours are staffed to best meet the more urgent, emergent needs of the community. 

Rural Wisconsin community tax assessment committees are soon surprised to discover the transformed locations are not just a Doctor's office and now meet the legal property tax exemption.  Outpatient staff are surprised to learn they must re-educate to best perform the higher level interventions.  Inventory is managed much more accurately with technology.  Transitions to more advanced care is explicitly defined within an evolving evidence based process.

Medicare AWV Annual Wellness Visit and ADL's

Medicare is a valid pay source for (AWV) the annual wellness visit.  The AWV annual wellness visit is a preventable approach which is in addition to the routine physical check up.  Medicare demonstrates the importance of having the AWV annual wellness visit by waiving coinsurance, part B deductible, and copay costs for the card holder.

The AWV annual wellness visit includes PPPS providing personalized prevention plan services.  This involves the clinic staff assessing the ability of the elder to successfully perform the activities of daily living on their own.  This is listed under the Medicare certified provider guideline elements of acquiring the beneficiary history and review of functional ability and level of safety. 

The AWV annual wellness visit with PPPS providing personalized prevention plan service regulation includes two components.  One component is providing the assessment and the second is referring to community health programs (www.homecarepath.com) designed to meet the need. 

Once the Medicare certified provider has documented the assessment and referred the needs to the community organizations a claim can be submitted.  For an initial appointment for an annual wellness visit the provider lists HCPCS code G0438.  With the subsequent  wellness visit the HCPCS  code is G0439. 

This regulatory connection encourages the clinic (primary care provider) staff to participate in tasks of care coordination beyond the building walls.  This is strengthening a community structure linking providers of long term care services to the families with aging loved ones needing help.

Chikungunya Virus Coming State Side?

Chikungunya is a virus transmitted by the mosquito.  People are infected by a bite from a Chikungunya virus carrying mosquito. 

First symptoms of Chikungunya virus can appear 3 to 7 days after the infected mosquito bite.  An elevated body temperature (fever) and joint pain show up first.  This can be accompanied by a  head ache, rash, and or general muscle ache.   Reports indicate initial symptoms can persist about 1 week.

Symptoms are managed with over the counter pain medications and extra fluids to prevent dehydration.  People are encouraged to see a physician.  Clinic staff can send a blood sample in to a (CDC) Centers for Disease Control recommended Bio-Safety Level 3 lab for analysis. 

Some people have reported the joint pain can remain for months.  Prevention involves avoiding mosquitos, using mosquito repellant. 

Current Chikungunya cases came in with travelers who were bit outside the United States (Haiti- Caribbean Islands etc.) and returned to be diagnosed State side.
