Collaborative Page To Reduce Hospital Readmissions

Information technology is being challenged to establish a software application capable of producing an immediate computer screen formed from recorded health care system contacts through out the varied medical providers locations.  This would be a collaborative page of an individuals medical chart. 

This would give an assessing medical provider the ability to compare assessments against past diagnosis, previous tests, prescriptions, and historical record.  This is real time, community wide, immediate review of an individuals health data. New York's Health Information Exchange is an example of developing and utilizing this type of technological application.

The collaborative page is a technological advancement that could improve the quality of a medical providers assessment process.  This is an essential piece of the hospital providers responsibility to determine the need for an inpatient stay.

The collaborative page facilitates digital communication that can be viewed any time through a pass word protected , cloud based portal , that can comply with existing HIPAA privacy laws.  Home Care Path staff expect to see a technological solution designed to unify immediate access to all medical information recorded with past provider visits.
