The entire community is tuning in to the State of Wisconsin transition to the new Family Care program. Home Care Path is structuring a template to aid in communicating where the participants are in the transition. The new Family Care program will pull 55 thousand beneficiaries in to a brand new system that integrates long term care, acute care, and primary care.
The new Family Care beneficiary will need to know:
- How to access service
- The availability of continuity of care to best meet changing needs
- How the quality of the care is measured
- How those with both Medicare and Medicaid benefit from the service
The new Family Care provider will need to know:
- How to contract with the authorized Integrated Health Agencies
- Staffing level and training required to evidence service readiness
- Mandatory reporting to demonstrate care assurance
- Range of reimbursement rates
The new Family Care Integrated Health Agency will need to know:
- Details of claims process and payment schedule
- Number of contracts per discipline to demonstrate network adequacy
- How payment is defined across measures of performance
- Capitated rate benchmarks along continuum of care
This structure is designed to assist Wisconsin participants understand where the process is within this ongoing new Family Care program transition. From a community health view this is the biggest care transition the State has ever faced and the risk is increased hospital readmissions. Understanding the above aspects of the new Family Care program transition is expected to help reduce individual incidence of re-hospitalizations across the State of Wisconsin. This is an intervention on the community level designed to strengthen the hand off and maintain health services while observing fiscal integrity.
