HCBS Home and Community Based Service Programs have submitted plans to CMS demonstrating the activity moving (MCO) Managed Care Organizations, Consumers, and Community Providers in compliance with HCBS mandates.
Understanding this change is thought to reduce a "displacement perception" that can accompany whole communities shifting with policy mandates. MCO Managed Care Organizations have been instructed through application of assessment tools to communicate needs that match task to an average time. This flattens rates reimbursing providers at a consistent amount per task. The average time for bathing is twenty minutes you will be reimbursed for twenty minutes each day your member has their bath. Similar for all recipients of service often called members.
Vocational supportive settings will be reimbursed to deliver a temporary service designed to assist members finding employment within the community of residence. Member centered plans will contain a defined length of time with lessening frequency as progress plateaus . This can be perceived as a loss with members who have long term attendance with area workshops.
In the past residential providers were encouraged to make rooms in their dwelling available for members to have a place to live. Two members sharing a room was thought to benefit all. The HCBS mandate will reduce reimbursement for a member sharing a room with another member. The HCBS mandate would prefer members residing one to a room or apartment. Residential providers delivering a service that supports two members in the same room will see their reimbursement drop.
Transportation will be shifted to family and providers. Serving families across the rural Wisconsin landscape has proven to be a transportation challenge for State Waiver Programs. The HCBS mandate will limit transportation to medical appointments and pharmacy visits. All contracts forged with CMS authorized Managed Care Organizations will include transportation as an unreimbursed expectation of provision.
What this means is that some providers will pull out, some new providers will step in, and waiver members will participate in some switching to try to stabilize their living condition with these changes. Expect those authorizing service to be slightly behind the actual rate of change within the community. This will look like authorizing teams asking for a much more rapid transition in to a community providers service. Can you start service for this member right now.
