New HCBS Rule Fast and Hard Community Push

To ensure that individuals receiving long term service and supports through home and community based service (HCBS) programs under the 1915 (c) 1915 (i) and 1915 (k) Medicaid authorities have full access to benefits of community living and the opportunity to receive services in the most integrated setting appropriate.

Home Care Path embraces a shared care coordination approach to serving the populations needs with the State of Wisconsin transition plan.  Shared care coordination is most effective when employing an inter-professional collaborative process which is best defined with the PATHALONG copyright model.  PATHALONG improves health care delivery in Wisconsin.

Providers are called on to participate in person centered planning that instills personal control and is outcome oriented.  Providers share delivery in services that are available to the rest of the community.  Integrated is the new specialness that flows from a genuine understanding of the nature and quality of the  individuals desired experiences.

Home Care Path encourages readers to watch as Care Management Organizations and community providers adjust delivery to best comply with the new HCBS ruling.
