Equivafund is a destination web application designed to deliver a fixed route for the unplanned health condition requiring the elder to shift funding sources. This helps families of sick elders who no longer meet inpatient status and must go home but still need a lot of help. This is a shift from public (Medicare) funds to private payment.
The elders needs have reduced to personal supportive care and no longer indicate a skilled medical provision. The elder must discharge from the hospital or nursing home and find rapid access to a personal supportive care provider.
Wisconsin hospitals and nursing homes can share discharge staff while serving a several county geographical area. Discharge departments have limited time and staff to assist with each discharge. Some discharge staff struggle with the conversation that can require family to pay out of pocket for needed care. Equivafund establishes a guideline that supports the inpatient provider, the patient and family and the community provider with this consistent need.
