The primary MD writes an order for a referral to another outpatient specialty provider. The nursing staff report the outpatient specialty provider will call to give you your appointment on the phone cause their office cannot schedule for another. A week goes by...

The elder receives in the mail prior to the next specialty provider visit the paperwork to be filled in. There is the 4 page patient initial history questionaire, the 2 page patient registration form, and the reminder to contact your insurance with any prior authorizations.
The elder cleverly notes that a clerk in the exact same hospital building less than 7 steps apart is asking for the identical information being stored as an electronic medical record for this patient in the computer 7 steps away.
The senior explains that being quite ill with this ongoing medical process she is not necessarily the most accurate source of information. The elder expresses frustration with the obvious disconnect between health care providers working within the same network.
