The Robert Wood Johnson Foundation (RWJF) has as a mission to improve the health care of all Americans.  The Robert Wood Johnson Foundation Care About Your Care is an initiative to improve care transitions as a way to reduce avoidable hospital readmissions. 

Wisconsin's PATH ALONG model utilizes copyright content to improve care transitions.  PATH ALONG is a strong arm of Home Care Path  www.homecarepath.com a community based organization commited to performing a service that supports the changing needs of an older population. 

PATH ALONG is an evidence based model with 3 defined services to help the hospitalist fulfill their continuity of care role.  PATH ALONG helps seniors during the hospital stay and outpatient treatment visits.  The focus is on care filled transitions.

The PATH ALONG model provides a simple assessment set that would indicate a person would benefit from a referral.  This improves access to an advanced delivery of supportive care.  Model structure incorporates individual preference as a way to enhance participation and demonstrate a measure of care assurance.

Reducing hospital readmission rates through assisting a patient with transitions of care is a big part of what Wisconsin's PATH ALONG model does.  Helping a hospital maintain fiscal integrity and retain the ability to perform needed medical services at their current location.  PATH ALONG is a model created to give the Wisconsin inpatient provider a quality referral source as response to federal regulations that reduce reimbursement rates for hospital readmissions.
