PATH ALONG Care Transition Includes Supportive Care

PATH ALONG Revolving Door Syndrome Dartmouth

The Dartmouth Readmissions Report is out.  The Readmissions Report is an ongoing project through The Dartmouth Institute For Health Policy and Clinical Practice located 35 Centerra Parkway, Lebannon, NH 03766. 

This report separated readmissions by two characteristics.

1. First admission involved surgery

2. First admission involved a (CHF) medical condition

Current values indicate an increased awareness that readmissions (Revolving Door Syndrome) are a costly undesired aspect of todays health care delivery.  Long time community health care planners have recognized that part of the problem is a lack of understanding by the user population.

The Dartmouth Readmissions Report demonstrates that higher readmission rates correlate with higher admission rates.  So the greater the hospitals user population the greater the chance for an elevated readmission rate. 

The study involved an interview process that seemed to indicate that severe illness and fatigue associated with a hospitalization reduced the patient's ability to retain important information.

PATH ALONG the Hospital Paperwork

One senior describes entering the hospital under the green awning and going to the first door on the left.  The senior had pre-registered over the phone with a clerk working in another State.  The senior attended the outpatient procedure and left with instructions to make an appointment with the primary MD as a follow up. A week goes by...

The first thing the primary MD asked at the follow up visit was what did the outpatient staff say with your last procedure.  The elder tells the primary MD an abbreviated version of their memory of the experience. 

The primary MD writes an order for a referral to another outpatient specialty provider.  The nursing staff report the outpatient specialty provider will call to give you your appointment on the phone cause their office cannot schedule for another. A week goes by...

The clerk calls for the outpatient specialty provider and schedules the next appointment.  This appointment is to take place approximately 7 steps from the first door to the left under the green awning entrance.  A week goes by...

The elder receives in the mail prior to the next specialty provider visit the paperwork to be filled in.  There is the 4 page patient initial history questionaire, the 2 page patient registration form, and the reminder to contact your insurance with any prior authorizations. 

The elder cleverly notes that a clerk in the exact same hospital building less than 7 steps apart is asking for the identical information being stored as an electronic medical record for this patient in the computer 7 steps away.

The senior explains that being quite ill with this ongoing medical process she is not necessarily the most accurate source of information.  The elder expresses frustration with the obvious disconnect between health care providers working within the same network.
