Home Care Path is leveraging technological community connections to strengthen access to senior living information.  Wisconsin's Home Care Path employs a lean operating model implementing progressive industry upgrades to help sustain elder care services.

Wisconsin is experiencing a shift in typical applications secondary to 2015 budget proposals.  Communication and collaboration are vital services required to best help the community transition through this change.  Home Care Path identifies collaboration as their core service. 

Home Care Path has registered nurses who conduct independent personal supportive care assessments which creates a family friendly process to inform the elder's care. You can then determine the arrangement of support that will best meet your senior's needs. This simplifies coordination which can involve family, friends, public programming, and private agency. Home Care Path facilitates access to a group of care givers who can lend immediate support. 2015 rates:
RN Assessment 39.00 per hour
Care Givers 20.00 per hour
