Home Care Path staff would recommend:
- Health Insurance Plans contract with multiple services across the State to perform independent personal care assessments
- All contracted personal care assessors utilize the same assessment instrument
- Portion of the assessment includes a face to face visit in permanent residence
- Frequency is annually or with significant changes in status
- Same assessor could perform assessment and re-assessments
- Assessment provider is not certified to perform and be reimbursed for medical assistance personal care
- Case management and coordination of service is different than the independent personal care assessment process
Home Care Path is developing an independent personal care assessment service designed to facilitate the practice of health insurance plans authorizing local personal care providers to serve Medicaid beneficiaries . With the State wide expansion of service multiple providers will be needed to deliver an acceptable level of care assurance as a measure of quality. This is a significant piece of implementation with focus on variables to indicate improved over all outcomes. The assessment is utilized to inform care while lending the application to best evidence driven upgrades.
