The community based organization's approach to developing a person centered plan must consistently involve:
1. Directed by the consumer being served
2. Includes assessment of personal/supportive care long term needs
3. Includes participants the consumer has freely chosen
4. Assessment of individualized preferences9
5. Lists goals the consumer has selected
6. Demonstrates integrated operation to improve access to available services within the community
The Centers for Medicare and Medicaid have notified staff within the Department of Health for participating States to submit a plan that utilizes bench marks and time lines to communicate the activity to achieve compliance by March 17, 2019. This entire initiative exists to facilitate community living. This moves public funded services from a high rate (hospital, nursing home) of delivery expense to a (home with supportive care) lower cost setting.
The person centered plan engages the consumer while including a larger share of the responsibility for achieving the desired outcome. This includes the consumer in measures that reduce risk and improve quality. This is in no way, shape, or form, as easy as it may sound- the process involves ongoing contacts across multiple community providers.
