Home Care Path (www.homecarepath.com) is a personal/supportive care community organization founded on the principles of including family, and community providers in the ongoing planning and care delivery process. This is the future of elder care across the United States. Staff scheduling that accommodates for impromptu family visits is an important benefit to the Wisconsin seniors we serve.
The Institute for Patient and Family Centered Care has been around since 1992. Located on 6917 Arlington Road, Suite 309, Bethesda MD 20814. The Institute of Patient and Family Centered Care shapes regulatory policy and supports change on a national level. An important initiative "Better Together Partnering With Families" guides health care centers with adapting regulations that welcome the family 24 hours per day in to their hospital. The idea of pushing family out during certain hours will soon be gone.
Hospitals and Nursing homes across the United States are being encouraged to engage family and partner with community providers when serving area citizens. This is an important strategy for maintaining a safe quality of care delivery while reducing the expense associated with hospital readmissions.
