Analytics Show PATH ALONG A Utilization Enhancer

An initial review indicates the Wisconsin PATH ALONG model is effective in improving utilization.
The Wisconsin PATH ALONG model provides inpatient staff and consumers with access to supportive care services.   This is important cause Wisconsin is primarily a rural setting that has pockets that can be a challenge to find service.

Inpatient payers are consistently separating skilled care from tasks of supportive home care to manage revenue out flow.  This is described by the patient population as tightening the home care benefit.  Tasks of supportive care typically require private payment, a long term care policy, a non-service related VA pension, Family Care, Wisconsin Partnership, or the IRIS program.

The supportive home care industry is increasingly called to participate in consumer driven cost comparison data structures.  This helps deliver similarity in the hourly rate being charged for an in home supportive care staff to help your loved one.  Flattening rates can change a supportive care service providers ability to retain an adequate monetary risk reserve which allows them to continue to serve the communities growing needs.

The supportive care industry will discover use (utilization) drives revenue.  The Wisconsin's PATH ALONG Model App  http://appsmakerstore.com/appim/j6kcdet8xvwk4s  increases utilization for the Home Care Path  www.homecarepath.com supportive care provider.

The strength within the Wisconsin PATH AONG model is the immediate inpatient access.  A hospitalized consumer (need) can access the PATH ALONG Model through a free app, which includes a connection to a supportive care staff at an hourly cost of 20.00.

The Wisconsin PATH ALONG model indicates a value through increased utilization of supportive care for a senior care industry leader Home Care Path.

Can MA Medical Assistance Medicaid Help Pay For Needed Care

Medicaid (MA, Medical Assistance) is a helpful public program jointly funded between the Federal and State Government.  Medicaid exists to assist in paying for skilled health delivery and many other medical expenses for individuals who have minimal assets and inadequate income to pay for these expenses.

In 2013 long term planners in Wisconsin used the following quick look at eligibility:

-no more than 2 thousand dollars in cash or savings

-home is exempt if the individual intends to live there

-one car with market value below 4500.00 is exempt

-life insurance is exempt if face value of all combined is less than 1500.00

-household goods and personal effects were not counted as of 2013

-prepaid funeral expenses exempt up to 3 thousand dollars

An individual would go to the County (ADRC) Aging and Disability Resource Center and request a benefits eligibility review to start the process.

Memory Loss Dissecting Data To Define Primary Risk

In this example Home Care Path utilizes an analytic structure as a means to get beyond the predictive capacity invoice data has afforded the supportive home care industry. Inherent in the research observations lie a primary risk often invisible to the evolving standards and rules directly related to the medical communities push of acuity down the continuum of care.

The symptom of memory loss is identified from the chronic condition.  The service associated with memory loss is in home supportive care.  The memory loss manifests from a chronic progressive disease process.  An individual with memory loss due to a chronic progressive disease process will continue to need more support from a care giver.  The individual will always need more help from the caregiver, not less.

This speaks to the amount of variability across individuals in terms of the service need.  In home supportive care addresses the initial risk (time alone) with an average cost of 20.00 per hour to deliver the service.  The symptom of memory loss carries a risk which can be characterized as a measure of time alone.  Simply, increased time alone equates to increased risk.

The symptom of memory loss can take advantage of any change in service provision directly related to the exhausted private funding of supportive care.  Because the individual continually needs more service the family frequently exhausts the private funding sources.  This defines the primary risk as the loss of supportive care service as the individual transitions from private to public funding.

This identifies how the family should compare supportive home care organizations when seeking to help a loved one with memory loss.   The primary risk is addressed as a continuity of supportive home care service in the time period the individual must transition from private funding to public funded service.  Home Care Path encourages readers to anticipate the need to transition from private funds to the use of public funded programming.

Value Based Health Care Delivery

The newest strategy to spread risk across the health care participants is the shift to value based.  Value based delivery inserts quality enhancing attributes in to structure designed to drive reimbursement factors.  This list of attributes constructs a value added delivery structure.

Care Coordination
Patient Education
Preventive Services
Wellness Monitoring
Telemedicine Email and Phone Communication

Value based delivery seeks to insert the above non reimbursable tasks within a structure of reimbursement that rewards desired outcomes   The achievement of the outcomes involves a delivery practice that documents an ongoing participation in the above list of attributes.

These tasks support the provider population by bringing more of the patients defined needs under management.  This extends the medical providers reach by reducing the gap between developing needs and receiving treatment.  This ongoing approach to managing human disease flattens larger stagnant piles of risk. This approach brings the patient , payers, and providers an equal distribution of risk.

Home Care Path encourages readers to watch as systems of medical reimbursement begin to financially reward structures of delivery that produce quality care for a lower financial investment.

Bring On The E Visit Fast Track To Medical Delivery

E visits can be done with a video camera or just a telephone.  This saves a sick patient time and money by not having to travel to the clinic office.  E visits are not a face to face encounter with the medical provider.

E visit programs embrace conditions where increased levels of scientific evidence have established best practice leaving technology to safely recommend the most appropriate course of therapy.  Typically the prescribing provider will have had a face to face with the patient before conducting E visits.  In this fashion the technology is used along side the medical provider who carries the education, experience, instinct, and judgment to help support the decision making.

The patient to provider encounter would involve evaluating the submitted data in relation to the change of condition and medical history.  Current research suggests that the benefits to E visits exceeds the risks.  Often a medical provider with a full  clinic schedule can accommodate E visits.

The payer (insurance plan) saves money as well.  Two data points compare the cost to the insurance plan.

E Visit reimbursement for symptoms of a urinary tract infection  $74.00
Office visit reimbursement for symptoms of a urinary tract infection $93.00

Home Care Path reminds readers to expect to see the E Visits being offered through your clinic medical provider soon!

Migration 2014 fairly Large Scale Health Care Center

Migration can be described as a regular journey under taken by humans who are called patients.  Patients migrate from delivery environment to delivery environment through a health care center that clusters providers who combine treatment and care with the promise of healing.

A physical functional loss or cognitive change instinctively triggers migration.  The PATH ALONG App is a navigation tool available to the migration stream. Try it for free
