Wisconsin Not Ready For Health Insurance Exchanges

Wisconsin, like many States has indicated it is not ready to implement the coming Health Insurance Exchanges.  Who runs the Wisconsin Health Insurance Exchange , State, Federal, or Private will not matter to the individual consumer.  Wisconsin citizens see this as an opportunity to flood the States economy with dollars.

The Wisconsin Health Insurance Exchange will be required to offer at least two national health insurance plans.  Wisconsin consumers can expect access to health insurance choices to compare and contrast very efficiently, and quickly.  Information screens will indicate eligibility for health insurance tax credits immediately. 

To Wisconsin this means many more people will be buying health insurance which will dramatically change the existing insurance market place.  A more viable market with more people purchasing coverage, means more insurance carriers will want to craft the products that can sell best.

As we read this non profit cooperative health insurers are getting loans from the Federal government to set up programs that will become operational in 2014.  Imagine a Wisconsin Health Cooperative getting a loan from the Centers for Medicare and Medicaid Services to establish a low cost, high quality health insurance product for the population of Wisconsin.

The Health Insurance Exchange adds transparency to the financial aspect of accessing needed health care service for Wisconsin citizens.  Organizing coverage levels with advanced technology to facilitate a comprehensive consumer satisfaction.  The system becomes user friendly and the product knowledge has an immediate, pragmatic communicative effect.

Genetic Connections To Alzheimer's Becoming More Visible

The New England Journal of Medicine published study TREM2 Varients In Alzheimer's Disease recently.  This study strengthens the correlation between brain inflammation and an increased risk of Alzheimer's disease.

This is important now that data indicates Alzheimer's disease effects approximately 5.4 million Americans.  The undesired symptom of Alzheimer's disease being the short term memory loss. 

Genetic mutation  R47H is a variant that reduces the white blood cells ability to get rid of plaque forming protein (beta amyloid) present on cells with the bodies inflammatory response.  This plaque has residence in the Alzheimer's disease manifestation of symptoms. 

To add to the confusion not all that have the R47H genetic mutation variant will develop Alzheimer's disease.  The more researchers can comprehend about the mechanisms that contribute to Alzheimer's disease, the better Science's chances of developing effective treatment.

Sauk County Care Transition Coalition

Home Care Path participated in the Sauk County Care Transition Coalition.  This is a local response to Federal (CMS) policy reducing reimbursement for hospitalization when a readmission occurs in less than 30 days.  The Centers for Medicare and Medicaid have forged policy that encourages hospital's to focus on discharge planning to prevent costly readmissions.

The Sauk County Care Transition Coalition bring together a diverse group of organizations who function to enhance the health and well being of the community.  This is a call to modernize collaboration pushing the independent practise in a bubble strategy toward an interdependent cooperative approach.   This calls for a frequency of communication that tightens connections beyond the simple referral process.

This verbalizes the notion that many area providers will serve an individual person at the same time.   Multiple providers participating in the care of one individual.   This meeting gives some shape to improving discharge planning and it comes in the form of cooperation.

SHOW Survey Of The Health Of Wisconsin

Survey (SHOW) of the Health of Wisconsin is funded by the Wisconsin Partnership Program and facilitated by the University School of Medicine and Public Health.  An office is staffed on 6602 University Ave, suite 5, Middleton Wisconsin 53562.  All collected information is private, published findings do not include the individual. 

SHOW began work in 2008 with a State wide focus and annual follow up.  Ongoing participation in a comprehensive plan to formulate research based policy designed to assure health initiatives are addressing the populations needs. 

There are four primary avenues for annual data collection.  The in person interview, the paper questionaire, the computer assisted screen, and body and lab values.  The survey approach mimics the National (NHANES) Health and Nutrition Examination Survey conducted by the Centers (CDC) for Disease Control. 

Collected data is recorded and reviewed.  The review includes comparison of the data to existing stores of information.  Defined program measurements can be weighed against local, regional, and national trends. 

Rather immediate results will indicate a proper level of resources which relates to the care assurance role.  Fiscal determination related to the provider approach directly measured by the effectiveness of the most current interventions.  Simply, is the money we are investing improving the health of the community.

Initiating The VA Aide and Attendant Claim

You are helping a veteran who has experienced a change in functional and cognitive status requiring additional aide and attendant time to remain safe at home.  You meet with a benefit specialist working out of the (ADRC) Aging and Disablity Resource Center.  The County benefit specialist determines the veteran is eligible for assistance per funcitonal eligibility assessment form, but is over monetary assets, so does not qualify for help. 

The veteran is turned down by the county for medical assistance programs with the instruction to return when the bank balance falls below 2 thousand dollars.  The veteran now locates a (CVSO) Certified Veterans Service Officer in their County of residence.

In the initial meeting with the Certified Veterans Service Officer  the agency working with the veteran provides the CVSO with a copy of the plan of care.  The plan of care demonstrates the relationship with the aide and attendant in home provider, the primary medical doctor, the pharmacy and specialty providers within the VA hospital health care setting. 

The family of the veteran is given The Pension Screening Form with instructions to list all out of pocket medical expenses.  The family must produce a copy of discharge papers called the military separation papers.  This is the beginning to determine if the VA claims process would benefit this veteran.

VA Working To Update Schedule Rating For Disabilities

The Department of Veteran Affairs in cooperation with The Government Accountability Office are working to update (VASRD) the VA Schedule For Rating Disabilites.  The past assessment process relied on the degree of medical severity as evidenced by visible symptoms as the disease progresses.

The VA is challenged to formulate a modern comprehensive approach to assessment utilizing current data from  The Internal Revenue Service and The Social Security Administration.  Mining collected information to determine the effect an impairment has on income. 

Rating formula's focus on functional impairment when revising criteria for categories of disability.  This incorporates a more individualized approach recognizing two veterans with similar medical diagnosis may have different levels of impairment.  This broadens the providers view of the impairment establishing some precision to the measure of functional ability.

This is a needs based strategy popular in the managed care model.  Given the changes in technology, needs can be met utilizing an option the veteran finds most helpful.  Resource allocation is no longer one size fits all, but what will actually benefit this individual veteran in their daily activities. 

The Department of Veteran Affairs is challenged to use the analyzed information from large systems of collection to develop research based policy.   The policy will guide an assembled infrastructure capable of organizing an update to the process while continuing to serve the veteran population.  Policy must maintain existing service levels while incorporating the updated change.
