Changes in risk tolerance felt across elder care industry

As benefits tighten the tolerance for risk changes.  Wisconsin is proactively altering their delivery to better manage the outflow of Medicaid money in the face of an increasing user population.  The Family Care and IRIS public funded programs are being folded in to a more medical focused delivery. 

This change will include the need for an individual to present with a high level of severity and intensity for skilled medical care to be eligible for a service that can be reimbursed.  The community and family will easily recognize the senior needs more help before the elder can be qualified as eligible for public program paid help.  The legislation and regulation is informing families and communities they must participate in more of the needs an aging population can be expected to manifest.

The Deficit Reduction Act of 2005 was to save the Medicaid fund by shifting more of the cost of long term care to families and inpatient (nursing home) providers.  To compensate inpatient providers were raising the cost of their service to off set any non- payment.  Some States have enacted laws making adult children liable for financial support of high medical need parents.  Estate recovery programs now charge families on the back end for required care. Communities and families now share in a greater portion of the risk.

Communities and families are confronted with the need to shop for service delivery in a category - elder health and well being,  we have never shopped in before.  These health care delivery laws create regulations that require communities and families to find pragmatic elder care solutions.  The far end of the undesired outcomes can be listed in the two levels below:

Communities and families are being expected to participate in a larger portion of an elder family members needs.  Understanding this new top down expectation can move local planners to embrace the solutions required to maintain the perception of a tolerable level of risk.  The availability of stable support along the continuum of aging will be an essential piece of planning to strengthen local economic outcomes.
