Long term care coordinators no longer consider this the best option. This leaves the family with a time consuming round trip that often results in less family contact. Worse, the senior is exposed to the disruptive acclimation process only to be uprooted and moved again when the desired assisted living facility lists an opening. Two major moves in a relatively short period of time can be traumatic for some individuals.
Thankfully Home Care Path is testing a service that will support the seniors personal and supportive care needs in their existing dwelling until the desired assisted living location becomes available.
Currently the team is delivering support for an elder male who is waiting for his room which will include a private bath room to open up. Having his own bathroom was identified as being very important.
The team is working with the selected assisted living facility which includes a series of visits to help with the seniors transition. This provides the elder and family extra time to arrange for the change (pets, plants, mail, medications) that comes with a geographical move.
