Hospice and Skilled Medical Home Care Social Workers Resource

Skilled medical home care and hospice are typically reimbursed through the Medicare fund which contains regulations that define benefit limits.  Both skilled medical  home care and hospice require a doctors order, and the person to have a Medicare card.

The doctor writes an order indicating the beneficiary meets the medical eligibility requirements to be served by the program.  This essentially means the elder needs skilled medical workers to help them in the home.

Some seniors will be served by both these programs and suddenly their needs change and there is not a level of care that requires a skilled medical staff to perform this service.  At this point the social worker in the skilled medical home care or hospice program must go to the home and discharge the patient.

This is an opportunity for the social worker to offer Home Care Path as a resource.  Home Care Path has senior support partners who go to the elders home and perform the non-medical tasks the person needs help with.  Home Care path charges 20.00 per hour for an individual staff (senior support partner) person to come and perform pre-scheduled tasks.

Non-medical supportive care involves helping an elder in the same way a family member would if they could be there.  Being sure the elder has adequate food, fluid.  Access to freshly laundered clothing.  Medication reminders, help with hygiene and sanitation.  Help with memory loss and keeping safe.

It can be hard for the family to understand why the social worker from the program is discharging their loved one - but being able to offer an affordable replacement may be helpful.  Home Care Path has been successfully helping families since 2010. Simply call 608-432-4286 to begin service.

State Wisconsin 2 Regionalize Insurance Delivery 2 Stabilize Rates

The State of Wisconsin in their effort to manage health care cost is posturing as a large payer to structure the delivery of insurance in a way to stabilize rates.  This encourages participating insurance plans to accommodate a regional service area.

Past Legislative Budget Committee's working with the Wisconsin Department of Health Services have designated 4 service regions across the State.  Wisconsin Administrative Groups are working to establish health plans capable of serving one or more of these regions.

The value of the larger regions to the participating health insurance plan is access to both State employed workers and the Medicaid population.  With two or more options in each region the State Administrative Groups can improve the accuracy of utilization audits from health services procured within the region.

By establishing a true cost per region the State hopes to stabilize rates by assigning calculation ranges that produce a known profit margin for the insurance plan participants.  This lends transparency to the payment structure that supports the delivery of medical care across regions.

Digital technology can then track cost and more accurately identify how the fund created through the State workers grouping or Medicaid beneficiaries grouping is distributed within the region.  This is essential to maintain a viable medical operating system within each region.

The fiscal auditors will be expected to effectively communicate the insurance plans margin above the operating expense per regional calculation.  The fiscal auditors will be expected to effectively communicate the medical centers margin above the operating expense per regional calculation.   This is research that constructs the rate of distribution from the managed fund to the insurance and medical providers to keep the community healthy.

HR 34 The 21st Century Cures Act Speeds FDA Process

President Barack Obama signed H.R. 34 The 21st Century Cures Act in to Law on 12/13/2016.  This streamlines the approval process for drugs and medical devices at the (FDA) Food and Drug Administration.

The law specifically defines the paperwork reduction act and relief from restrictions on support for scientific meetings.  The law facilitates rapid data sharing giving researchers in similar biomedical projects immediate access to (NIH) The National Institutes of Health published findings.

The law calls on the structure of research projects to be embedded with stronger techniques to assure privacy for participants. This law adjusts past perceptions of balance between patient safety and faster access to treatments.  Some researchers are excited and others a bit nervous about the new pace that will be established for products produced by advanced research being marketed for human utilization.

Municipalities To Shift Cost of Dementia Crisis

The number of individuals experiencing memory loss related to dementia is growing nation wide.  The effective management of symptoms the disease of dementia can create is time consuming and costly.

Research indicates municipalities are implementing different approaches to help manage the cost related to dementia across the community served.  The municipal authority considers an ordinance to establish a monetary fine for preventable emergency service calls related to dementia crisis like symptoms.

The symptoms related to dementia are progressive in nature.  Even with an organized process of care the individual can experience crisis like events.  The crisis like events can scare most everyone involved and usually results in a contact of emergency services.

The emergency responders working with law enforcement may arrange an emergency placement.  This removes the elder experiencing the undesired symptoms of dementia to another location where care can be delivered.

In the past emergency responders could coordinate an emergency placement in a local assisted living dwelling where the elder is helped until the crisis like symptoms pass.  The family could take comfort in the knowledge that there loved one was close by.

With the implementation of an ordinance that establishes a monetary fine for preventable emergency service calls the local assisted living dwelling will no longer accept an emergency placement.  The assisted living provider rather than serving an immediate high need individual with the risk of a municipal fine will turn the emergency responders request for help away.

This greatly limits options and often results in the individual in crisis being sent to an available bed in a lock down unit in some part of the State.  This involves transportation and labor costs that may not have been listed in the annual budget.

Home Care Path research indicates the complex nature of the undesired symptoms of dementia can create crisis like events that cannot be properly served by one organization or specific discipline.  We are all in this together and it will likely take a community approach to effectively care for our aging citizens while holding costs down.

Understanding Medical Treatment Assisted Living Wisconsin Long Term Care

Home Care Path invests in supplying the community with information that can improve health outcomes.  Our thumb nail glimpse of the long term care continuum is shown below.  Understand this is presented in a progressive fashion.  So least expensive and least invasive to more expensive and more invasive.

 $ Least Expensive $

Technological Tracking (phone response) non-medical

In Home Supportive Care (help of another) non-medical

Assisted Living Dwelling (geographical move) non-medical

Skilled Nursing Facility (medical treatment) medical

 $ Most Expensive $

This is a condensed list of understanding how an elder's needs which can be created from a loss of physical function or cognitive thinking are managed in Wisconsin.  The first 3 on the list , technological tracking, in home supportive care, and residing in an assisted living facility all require the senior to utilize the medical services available in the community.

So the family and staff helping with the elder continue to assist the senior with attending scheduled visits and medical services.

Primary Medical Doctor
Hospital and Specialists
Skilled Nursing Rehab Facility

As an elder begins to participate in the first 3 services (technological tracking, in home supportive care, assisted living dwelling) their needs tend to require an increased frequency of medical system utilization.  This often looks like the chicken and the egg question of which came first.  So this means:

1. An elder using the medical system ( Clinic, Hospital, Nursing Rehab Home) more often can cause medical staff to suggest the senior get more help.

2.  An elder participating in the non-medical services (technological tracking, in home supportive care, assisted living dwelling) can cause staff to schedule more medical services.

This dynamic can become more pronounced in rural areas where an available service deficit exists.  In other words the need for help with elders is greater than the number of services available to help.

These non-medical services have emerged in response to the structure of regulations and cost that make up the medical nursing home industry.  For an elder to stay in a medical nursing home they must meet eligibility that is determined by a comprehensive assessment that signifies a medical need.  Along with this an elder is often required to pay a lot more for a medical nursing home stay.

The key to understanding elder care in Wisconsin is understanding that no one person or service can effectively manage the complexity of needs the aging process can manifest.  Home Care Path is structured on the idea of the importance of coordination, communication, and collaboration.  Home Care Path has a participating practice in non-medical delivery, senior housing, medical ancillary, and pet care.

Home Care Path Facilitates Wisconsins Senior Housing Market

Wisconsin has a rural geography with small burgs scattered along roadways.  These burgs (small towns) have started to develop larger real estate properties as senior communities.  Senior communities will embed residential property with access to both medical and non-medical services to support the elders daily activities.

The senior communities appeal to individual elders who are ready to simplify their life by down sizing .  Most geographical moves for a senior depend upon the sale of their existing Real Estate.  Home Care Path coordinates senior support partners who can come to the elders home and help for 20.00 per hour.  This has proven essential when the senior is waiting for the sale of their existing property.

Downsizing the homes contents involves developing an inventory and designating objects to be shipped to family and items to be sold.  Home Care Path has down sizing helpers who can assist the senior with this often emotional process.  Today's seniors have been finding success with online selling of their objects.  Home Care Path charges 20.00 per hour for an individual staff person to come and help the senior with the downsizing process.

Wisconsin Dells and the larger resort area is known for the availability of vacation rentals.  In this scenario the family converts the hard to sell residence to a vacation rental.  Home Care Path operates Condo Clean a service that sends staff post occupancy for light housekeeping, onsite laundry, and errands to stock the condo unit.  An individual staff person for 25.00 per hour performs the pre-scheduled tasks.  This allows a greater occupancy which can improve vacation rental profits.

Often the senior will have extended family that will purchase their dwelling if only their credit were better.  Home Care Path delivers Wisconsin Credit Repair Nerds service to help individuals improve the presentation of the information on their credit report.  This can improve access to funding options.

Home Care Path has always seen pets as our family members.  We have staff that can come to feed, water, and walk your pets for 19.99 per half hour.  This can be beneficial if the senior plans to be away from their home for a long time.  This is most cost effective when the senior has more than one pet.
