Reality Of Preventing Hospital Readmissions Wisconsin

The Affordable Care Act will schedule in the conditions (COPD) chronic obstructive pulmonary disease, total hip and knee replacements connected to Medicare reimbursement regulations.  The regulations make it clear hospital readmissions are no longer an acceptable part of the health care system.

Research indicates there are 3 approaches that seem to be effective in reducing the providers risk associated with maintaining a safe inpatient service.

1. A process of inpatient to community coordination.  Wisconsin's PATH ALONG is an original copyright model

Wisconsin Carbone Cancer Center Seeing Better

To more effectively treat cancer it is important to be able to better see the cancer cells.  Cellectar Biosciences INC has developed the (APC) Alkylphosphocholine tumor targeting molecule.  The University of Wisconsin Carbone Cancer Center has established a diagnostic imaging process that utilizes the Alkylphosphocholine molecule.

Given through an IV line the molecules flow throughout the body- crossing the blood brain barrier, and inserts into the surface of the newly developed cancer cells.  Cancer cells lack the enzymes to metabolize this fluorescent  ( Alkylphosphocholine)  imaging label. This allows providers to view small forming cancer cell clusters in patients that have zero symptoms and does not show up on a typical MD exam.

This is direct cancer cell imaging.  Providers can tell if they are viewing returning cancer tumor growth or just surgical scarring, recent inflammation with an infection, or residual treatment changes. The key to stopping this lies in the eradication of the cancer stem cells.  Viewing the cancer cells can give providers a sense of how their prescribed treatments are working to shrink the undesired cells.

Basin is a coordination service being delivered through Home Care Path https://homecarepath.sharepoint.com/Pages/default.aspx.  and is billed at 39.00 per hour.  A discovery of undesired cancer cells in the body can result in a lengthy list of changes that require the participation of an ill individual.  How to meet the increased frequency of treatment visits, diagnostic testing regimen, ongoing medication changes, and discipline specific provider contacts can prove over whelming.

Basin is a coordination, analysis, and consultation process designed to care for individuals in the community.  Strategies include a custom care plan, medication reconciliation, rapid access to in home staff, identify primary care rapid path, track monthly household service invoices, visually place the disease process within the health treatment continuum.  This strengthens the community members ability to participate in this ground breaking diagnostic and treatment process.

PATH ALONG Calibrate Inpatient Stations Going Forward

Utilization of the PATH ALONG App has advanced considerably this last year.
Publishing of the PATH ALONG App is viewed as positive because it broadens the inpatient providers options for engagement producing opportunities with a proven track record.

A top contributor of efforts to reduce hospital readmissions primarily available in hand held information on mobile devices and tablets is the PATH ALONG App http://appsmakerstore.com/appim/j6kcdet8xvwk4s.  Delivering a degree of organization to the incoming data consolidating an individual approach to a readmission free recovery.  The structure lends itself to a management strategy focused on the varied transitions of a hospital stay.

It appears the positive change is the result of potential strength in marketing of mobile devices such as smart phones and tablets.  Adjustments with supply - demand environment in the mobile information devices has given available applications like PATH ALONG more exposure.

Calibrating inpatient contacts serves to deliver a measure of prudent allocation of the patient and family resources.  The insight shores up the substantial ability of the entire operation to  produce healing and a readmission free recovery. As a senior care industry partner of the national health care system, the Medicare fund will likely experience measurable savings.

This favorable analysis of the efforts of the PATH ALONG program have invigorated the staff of Wisconsin's Home Care Path  https://homecarepath.sharepoint.com/Pages/default.aspx
 Very compelling.
