2016 "Linking" Trend Changing The Elder Care Theme

The hot new concept in elder care is called "Linking".   The linking trend is evidence of the message repeated by long term care planning committees -The Experience of Aging Does Not Display Needs An Individual Discipline Can Effectively Meet On Their Own - is reaching critical mass.

New Family Care IRIS 2.0 Divides State of Wisconsin In 3 Regions

The New Family Care IRIS 2.0 will divide the State of Wisconsin in to 3 regions.  The Department of Health Service Federal Waiver Request to the Centers for Medicare and Medicaid will indicate plans to have 3 different Integrated Health Agencies operating in each region.

The Federal Waiver Request will indicate Family Care IRIS 2.0 beneficiaries will be able to switch to a different operating Integrated Health Agency at any time.  To attract adequate numbers of operating Integrated Health Agencies for each region DHS will publish a Request For Proposal. 

The Integrated Health Agencies will be required to be licensed as a Wisconsin Insurance Company.  The Integrated Health Agency will be required to demonstrate evidence of a local presence with relatively easy access.

Wisconsin AB 788 Four Dementia Care Specialist Positions

Wisconsin Assembly Bill 788 authorizes funds for the Department of Health Services through the Aging and Disability Resource Centers serving Counties with a population of less than 150 thousand for a Dementia Care Specialist position.  The 2016 legislation funds 4 full time Dementia Care Specialist positions.

State of Wisconsin's Argument With ACA Lawsuit

The State of Wisconsin has just joined 5 other States in a lawsuit against (ACA) The Affordable Care Act.  The argument of the lawsuit has to do with unclear language. 

Because of unclear language:

- these States have to contribute to fees to cover Federal subsidies

- this provider fee is viewed as an unfunded mandate

- contracted participants (State of Wisconsin) were notified by a private entity appointed as administrator

The States now realize regulatory defined contractual processes facilitated implementation of a service the participants did not understand - unclear language.

An example to help clear this up would be Wisconsin's transition to Family Care IRIS 2.0 and the unclear transportation need.

Wisconsin Family Care IRIS 2.0 Transportation

One of the most difficult aspects of delivery is the transportation piece.  Wisconsin Family Care IRIS 2.0 beneficiaries tend to need frequent trips to varied medical providers to manage the symptoms of chronic disease processes.  Individuals with cognitive deficits - memory loss will require another (door to door transport) to accompany them with these trips from home.

Wisconsin is a rural geographical area with no access to taxi cab service in much of the road ways. 
Serving a State wide population is expected to create additional transportation difficulties.

Initially the State through a Federal Waiver will guide the Department of Health Services to assign transportation to the Integrated Health Agencies.  Through contract the Integrated Health Agency will mandate the provider to perform all necessary transportation.  The provider contract will list the inclusion of medical transportation.

The transportation expectation is unfortunately viewed by providers as an unfunded mandate.  Specifically the assessment process which determines the time and task the provider can submit a claim and be reimbursed will not capture the transportation cost.   This is the disconnect. 

Committed providers who adjust their practice to insure longevity will not have the resources to perform the transportation piece at the rate of reimbursement.  This will not be immediately understood by all involved with this State wide heath care transition.

Expect beneficiaries and family, Integrated Health Agency staff, employee's of the third party claims, and care givers of contracted providers to begin to blame each other for the difficulty created with this system deficiency.

This predicted system wide deficit will look like:

- State includes transportation in Federal Waiver request
- DHS explains transportation will be managed by Integrated Health Agencies
- Integrated Health Agencies include transportation in standard provider contract
- Participating providers question precisely how transportation is reimbursed

Baraboo ADRC Sauk County Gets Funds 4 New Position

The Sauk County Baraboo Wisconsin (ADRC) Aging and Disability Resource Center has received approval from the Board for another full time position.  The fifty thousand dollars will be assigned to hiring an Aging Program Specialist.  This is a piece of Sauk County's ongoing effort to demonstrate compliance with the County Aging Plan.

Wisconsin Assembly Approves 50k UW Alzheimer's Research

The Wisconsin Assembly has approved a bill that provides 50 thousand dollars for the Alzheimer's Disease Research Center at the University of Wisconsin in Madison.  The focus of the research will involve existing stores of blood and spinal fluid samples.

The plan is to leverage technology and imaging to identify and name abnormal proteins in the blood and spinal fluid samples.  The presence of these newly named abnormal proteins in the blood and spinal fluid of an individual who's brain is known to have Alzheimer's disease is significant for crafting effective treatments.
