Wisconsin Act 10 Change Health Care Delivery

Is the health care delivery implications of Act 10 a symptom of the larger change taking place in the Wisconsin health system?   The Act 10 a 2011 Wisconsin Law that ended collective bargaining and fragments union operation for State employees. 

In 1990 the (ADA) Americans With Disabilities Act requires businesses and places of public accommodation to be accessible to folks with physical, sensory, and or mobility problems.  This encouraged new or remodeled buildings to accommodate individuals with functional deficits.  This is paving the way for individuals with long term chronic (nursing home residents) disabilities to reside in the community. 

The State of Wisconsin assumed authority for the delivery aspect of medical assistance paid community care.  This lifted authority from the individual Wisconsin Counties and placed it with State administered (MCO) Managed Care Organizations.   The Managed Care Organizations served multiple Counties now called State regions.

The Managed Care Organizations employed programs called Wisconsin Partnership Program, The Family Care Program, and IRIS I respect I self direct Program.  These programs coordinate services for individuals who once required a nursing home stay , to safely be served in their chosen community residence.   This is simply the State of Wisconsin managing the outflow of public health dollars by shifting care in to the community.  Care outside the hospital and nursing home is delivered at a lower expense. 

Health care delivery is consistently being pushed from the hospital and nursing home settings in to the community.  Along side this is the States initiative to stop paying skilled medical staff to perform non-medical tasks.  In the hospital setting this is communicated by legislators establishing laws that require medical providers to be paid the same amount regardless of the delivery setting the procedure is being performed.  If the procedure has been proven to be safely performed in the community clinic at a lower expense, the hospital will only receive the lower rate.  This is flattening care costs.

Act 10 fragments the traditional hospital staffing model and encourages fiscal representatives to determine through cost comparison core services and privatized services.   This is moving health delivery models to separate skilled medical care from non medical tasks employed with their operation.    The budget of a community hospital dependent upon a greater portion of public money (Medicare, Medicaid) to operate may need to privatize non medical  tasks to maintain financial viability.

This means medical care will be reimbursed at a community rate whenever deemed safe.  Very limited availability of public money for staff to perform non medical tasks.  This Act 10 appears to be a symptom of this larger change taking place across the health care delivery market.

Alibaba NYSE BABA the tech stock wth profit potential

Alibaba Group Incorporated the e-commerce provider has chose the NYSE stock symbol BABA and the IPO Initial Public Stock offering is expected to be scheduled for August 8, 2014.  Alibaba established T-mall for the brand retailers and Tao Bao to facilitate the consumer to consumer online sales.

In 2013 Alibaba produced more than 248 billion in online transactions.  Alibaba's strength is in the Chinese market with the expectation of 360 million Chinese consumers using the Alibaba service in 2014.

Alibaba employs a strong infrastructure comprised of proven international supply chains, advanced transportation and technology, with an expanding approach to the global e-commerce industry.  Traditional stock analysts speculate Alibaba's IPO could exceed 20 billion dollars.

Wisconsin Reinvent Long Term Care Insurance as a 529 like plan

The Wisconsin State Journal Newspaper Reporter David Wahlberg has revealed a plan to reinvent Long Term Care Insurance.  Ingrid Thompson is working with Wisconsin law makers Mark Miller and Robb Kahl to craft a bill that would facilitate a Long Term Care Fund similar to those authorized by section 529 of the Internal Revenue Code (1996). 

The State of Wisconsin would regulate the operation of the Long Term Care Fund.  Like the College Savings Plan, the proceeds from the Long Term Care Fund could only be utilized with qualified Supportive Home Care providers. 

This is important because the success of such a fund would be dependent upon the availability of (www.homecarepath.com) supportive home care providers willing to contract with the State plan administrators.  In order for the contributing population to benefit from their deposited money a strong supportive home care provider network would need to be functioning throughout the State.

Basin Medication Reconciliation Look Back Efficiency

One of Basin's interventions is medication reconciliation.  This is conducted on the heels of the PATH ALONG care transition process.   To help us understand this:

- as an inpatient the medical provider documents the undesired symptoms

- the inpatient provider prescribes the medication to treat the undesired symptoms

- the pharmacy fills the medications for the person upon discharge

- the person returns to the medical provider with the same undesired symptoms

Analysis of the individuals data seems to indicate to the Basin consultant that this patient is most likely to need additional  help with their medications.   Basin is a consultation service billed at 39.00 per hour.  Supportive care staff is billed at 20.00 per hour.

Healthy Seniors and Amyloid Levels study Solanezumab

Alzheimer's disease is believed to begin in the brain a decade before thinking, and memory problems are apparent.  Beta amyloid is a protein that builds up in the brain to cause the worsening symptoms of Alzheimer's Disease.

Solanezumab is a drug that binds soluble amyloid and helps clear this from the brain.  This helps prevent the amyloid from going on to build brain plaques. The hope is this will halt the Alzheimer's disease progression, preserve thinking, and memory skills.  This helps stall the mental decline and maintain greater cognitive function.

The National Institutes of Health and others will monitor amyloid levels and record cognitive test results with study participants.  About half of the 2100 participants will receive 400 milligrams of Solanezumab per IV injection every month for 18 months.

The A4 Study is anti-amyloid treatment in asymptomatic Alzheimer's.  Solanezumab is owned by Eli Lilly and Company.  Participants are healthy seniors age 65 to 85 years of age.  This is an attempt to divert the ingredients needed to assemble the development of Alzheimer's Disease, and stop the manifestation of undesired symptoms like memory loss.

Wisconsin's Epic Health Kit Apple and Mayo Clinic

A progressive patient engagement tool was revealed on Monday June 2, 2014 at the Apple World Wide Developers Conference in San Francisco.  Apple is developing (Health Kit) a support platform to facilitate the ability of individuals to upload health values directly in to their own electronic medical record.

This is cloud based data transport designed to connect Epic, Apple, Mayo Clinic, and the individual patient.  Patients and clinic staff will interact in a new way.  Current values would be available with the patients next face to face provider visit. 

This gives Health Kit users the perception of a more individualized approach to medical service.  This encourages the consumer to participate in their own ongoing preventive strategies.  The patient can better understand how diagnostic values contribute to their over all health and wellness picture.
